    THIS IS A CONTRACT POSITION OFFERING NO BENEFITS!

     

    The purpose of this contract is to provide outpatient assessment and treatment services to individuals with gambling disorders and to provide consultation to staff regarding pertinent gambling issues. Deliverables: The CONTRACTOR will provide gambling treatment services as staff, and otherwise do all things necessary for or incidental to the performance of work, as set forth below:

    Minimum Qualifications

    Minimum Qualifications:

    CONTRACTOR shall be licensed or under the appropriate board-certified supervision to maintain credentials under the Addictive Disorders Regulatory Association (ADRA). CONTRACTOR shall adhere to all requirements of licensure including those for obtaining Continuing Education Units (CEU's).

     

    CONTRACTOR shall provide compulsive gambling outpatient screening, supportive counseling and case management services to a maximum of thirty-two (32) clients at any given time an average of forty (40) hours per week, fifty-two (52) weeks per contract year. Total billable hours shall not exceed two thousand eighty hours (2080) in the contract year. Hours available for service delivery may be adjusted with the approval of the NEDHSA Executive Director or his/her designee to accommodate unforeseen circumstances, vacations, sick days, etc. However, in no case may the number of hours billed times the rate paid exceed the maximum contract amount.

     

    CONTRACTOR shall participate in ongoing supervision through the clinical services team and participate in all clinical team meetings/activities

    Problem Gambling Treatment Services:

    CONTRACTOR agrees to provide services for a maximum of 32 clients at any given time. Services shall be performed within accepted guidelines of the profession, region, and applicable licensing and legal requirements. No more than 20% of the caseload shall be located in correctional facilities.

     

    CONTRACTOR shall screen clients who are referred for problem/compulsive gambling treatment. Screening instruments shall be used as prescribed by the NEDHSA. Individuals who do not meet the eligibility criteria for treatment as defined by the instruments named above, then an appropriate referral to other community resources, including NEDHSA Clinics as appropriate, shall be made. Referrals to other resources shall be documented in assessment records.

     

    CONTRACTOR shall develop an Individualized Treatment Plan for each client within 72 hours of admission. Based on the information gathered through assessment, along with any other relevant information taken in the interview, the treatment plan shall be developed in conjunction with the individual receiving services. Treatment plans shall be reviewed and revised as required (every 90 days) or more frequently as indicated by client needs, and shall be followed consistently by staff. Treatment plans shall contain all elements described in current LDH licensing requirements for Addictive Disorders treatment including, but not limited to:

     

    • Client-specific, measurable goals that are clearly stated in behavioral terms

    • Realistic and specific expected achievement dates

    • Interventions/strategies/activities to help the client achieve the goals including frequency, type of service, therapeutic intervention, etc.

    • Complete, pertinent information related to mental, physical, and social need of the client

     

    Clients with assessments which reflect behavioral health issues, i.e. mental health and/ or substance use, shall be referred to the nearest NEDHSA clinic for evaluation for further treatment. Should a client be treated concurrently at an NEDHSA clinic for behavioral health issues, evidence of coordination of services and collaboration in the provision of care should be evident in the treatment plan and progress notes in the client record. Services provided shall include:

     

    • CONTRACTOR will provide individual and/ or group supportive counseling as outlined in the client's Individual Treatment Plan and provide continuing care services following treatment.

    • CONTRACTOR must show evidence of implementation of evidence based practices such as motivational interviewing and/or client centered treatment approaches, etc.

    • CONTRACTOR will provide educational lectures about identifying compulsive/problem gambling to NEDHSA clinic staff, other addiction treatment facility staff in the NEDHSA integrated care network, and clients during groups at these facilities at least two (2) times per year. Educational lectures shall include, but not limited to:

    o Signs and symptoms

    o Predictors and indicators of gambling addiction

    o Triggers and risk factors

    o How to avoid problem gambling

    • CONTRACTOR shall provide family counseling at the request of the client/family.

    Community Outreach:

    In the course of the contract period and at the request of the NEDHSA, CONTRACTOR agrees to provide at least five (5) educational opportunities, i.e., media presentations, educational presentations, etc., to the local community regarding prevention of compulsive gambling and the availability of treatment resources. CONTRACTOR shall document the times and places of such presentations and provide them to the NEDHSA with the monthly invoice.

     

    CONTRACTOR further agrees to make a minimum of ten (10) contacts (telephone and direct) per month with local community medical and social service providers in order to encourage referrals for treatment, which shall include a minimum of two (2) contacts who have not previously been contacted about gambling services available.

     

    CONTRACTOR shall provide evidence of coordination of services and community outreach with the Louisiana Center Against Poverty (LACAP) and Partners in Prevention, "Kids Don't Gamble, Wanna Bet?" program. This coordination shall include screening, supportive counseling, referral and linkage for parents of program participants who self-identify as having an interest in gambling addiction services.

     

    CONTRACTOR shall provide a contact sheet documenting who was contacted and the date of such contact. Contractor shall also participate in at least two (2) community activities hosted by each contractor.

     

    CONTRACTOR shall work with District Attorneys in the NEDHSA area to recruit referrals of individuals in need of gambling services who are on probation and/or parole.

     

    CONTRACTOR shall market the gambling screening tools to other behavioral health treatment agencies to determine gambling treatment needs and recruit referrals.

     

    CONTRACTOR shall research agencies and stakeholders that can assist in increasing referrals.

    Record Keeping:

    CONTRACTOR shall maintain records for person served that document progress or lack of progress based on NEDHSA policy and procedure. Records shall contain at a minimum, screening form and documentation of service provision, treatment plan, discharge plan, referral source, referrals to NEDHSA integrated care network and/ or other community agencies, and all case activity.

     

    CONTRACTOR shall ensure that the charts of all Participants completing treatment contain a Discharge Plan and a Discharge Summary. For those individuals who exit the program prior to completion, the CONTRACTOR shall make recommendations for continuing care and document efforts to refer for continuing care in the participant's record.

     

    The CONTRACTOR shall document in the participant's chart referrals made and/ or attempted.

     

    CONTRACTOR shall provide a copy of the Discharge Plan to each participant at completion of treatment.

     

    CONTRACTOR shall ensure that all rules of confidentiality pertaining to medical records are observed.

    The type and scope of services shall be determined by agency needs. A state agency email account will be created through La.Gov for the CONTRACTOR to receive and send confidential correspondence. The range of services to be provided shall include, but is not limited to:

     

    • All services provided are documented in client charts as the services are rendered.

    • Program notes shall be completed and filed in the client chart record within 24 hours of service.

    •Records of former/closed clients shall contain a discharge plan and discharge summary for all clients completing treatment.

    • Referral recommendations and efforts are documented in all client records.

    • Admission and discharge date on the frequency of gambling, use of alcohol, and use of drugs are collected and input in the prescribed database in a timely manner.

    • Contractor shall submit a Monthly Program Report along with invoices to provide information on a monthly basis for program referrals, admits, discharges, and community/ NEDHSA integrated care network referrals.

    • Sign in sheets document in-service training

    • Each client record shall contain tobacco and gambling screening tools.

    • Program notes shall be completed and filed in the client chart record within 24 hours of service.

    • Contractor shall submit a schedule each month of all planned events and update plan as needed to accommodate scheduling changes.

    • Contractor shall submit invoices for payment on a timely basis, by the 10th day of the subsequent billing month, to ensure payment deliver.

    • Referral recommendations and efforts are document in all client records.

    • Participate in ongoing supervision through the clinical services team and participate in all clinical team meetings/activities, as scheduled.
