• Setting: Home Health

• Location: Orange County

• Schedule: AM or PM

• Pay: Up to $55/hour (we offer weekly pay!)

RN Essential Responsibilities:

+ Conduct comprehensive patient assessments in the home setting

+ Develop and implement individualized care plans based on physician orders

+ Administer medications and monitor for side effects or adverse reactions

+ Provide wound care and manage dressings for various types of injuries

+ Monitor and document vital signs such as blood pressure, heart rate, and temperature

+ Educate patients and caregivers on disease management and medication adherence

+ Offer emotional support and guidance to patients and their families

+ Promote patient independence through teaching and rehabilitation strategies

+ Communicate effectively with physicians and other healthcare providers

+ Maintain accurate and timely documentation of patient visits and progress

+ Coordinate care with interdisciplinary team members including therapists and social workers

+ Travel to patient homes and adapt care delivery to diverse environments

+ Observe for changes in patient condition and report promptly to the care team

+ Ensure compliance with infection control and safety protocols

+ Assist with activities of daily living when necessary

+ Evaluate the effectiveness of nursing interventions and adjust care plans accordingly

+ Provide end-of-life care and support for patients and families when applicable

+ Participate in case conferences and quality improvement initiatives

+ Maintain professional licensure and stay current with continuing education requirements

+ Use electronic health records and mobile devices to document and communicate care

RN Qualifications:

• RN license

• Experience using OASIS preferred

1 year of RN home health experience preferred
