Mission:
Our mission is to offer people with disabilities the necessary supports in order to promote the personal values of individual growth, participation, contribution and capability.
Organizational Focus:
We provide services to individuals with developmental/intellectual disabilities living in Colorado. Admissions are for individuals with complex behavior and medical needs who may pose a risk to themselves or the community. We provide emergency treatment stabilization for people and provide support to the community service delivery system.
Overall function of the work unit:
The work unit exists to provide skilled nursing support and services for disabled individuals residing in group homes scattered throughout the Denver Metro area (mainly Jefferson County). The Wheat Ridge Regional Center (WRRC) exists to serve individuals who qualify for active treatment services as defined in Intermediate Care Facilities rules and regulations for individuals with Intellectual Disabilities (ICF-IID). The nursing unit exists to provide 24/7 services to these individuals who have profound and severe disabilities with a high level of medical needs, and/or have significant behavioral and psychiatric challenges. The focus of treatment is stabilization, increasing knowledge base, increasing knowledge of treatment options (i.e., medications), individual and group therapy, discharge planning, working with guardians, family members, and other support systems for individuals. Understanding of health-related and medical issues in individuals with intellectual and developmental disabilities. Individuals with developmental disabilities, physical handicaps, and multiple medical and psychiatric diagnoses require multiple needs in addition to mental health treatment and stabilization. Individuals are admitted for evaluation, observation, treatment, and stabilization. The goal is to return them to their optimal level of functioning and assist them in returning to the community as appropriate.
Description of Job
Position Summary:
This position has supervisory responsibility over one residential home. It is also to implement the Individual Program Plan (IPP) for each individual. This involves active treatment; medical and therapeutic services (including administering medications); protection from harm; rights; teaching self-help skills (such as personal hygiene); interacting with the individuals in all areas of feeding, toileting, developing social skills, transportation, and recreation; community participation and money management; recreation and leisure activities; and quality of life. Maintains home and equipment for a clean and safe environment. Evaluates the effectiveness of the IPP by measuring individuals’ progress; recommends changes needed to benefit the individual. Responds to a variety of situations, including medical emergencies and behavioral problems.
Position Duties:
+ Manages one home for individuals with intellectual/developmental disabilities and routinely makes decisions related to ensuring adequate supplies are on-hand, and decides which situations warrant alternate purchases to provide for the safety and welfare of the individuals.
+ Responsible for ensuring the Individual Program Plan (IPP) for all individuals in the home is being implemented consistently and decides what recommendations to make to the team about changes to the IPP.
+ In conjunction with the team, decides what residential programs are appropriate and evaluates their effectiveness. If not effective, this position determines changes to the program.
+ Determines and evaluates the effectiveness of residential programs.
+ Responsible for making purchases and submitting work orders to ensure the home is in good repair and clean. This position is responsible for determining documentation of several programs to ensure compliance with ICF/IID regulations
+ Makes decisions about compliance with policies and procedures, rules, and regulations by the staff they supervise.
+ Makes decisions to recommend appropriate progressive discipline for all the staff they supervise. This position determines and completes annual and mid-year performance management plan reviews (PMPs).
+ Determines and responds to Step 1 grievances if staff supervised initiates a formal grievance.
+ Observes the individual for side effects or problems that may arise medically and determines the effectiveness of treatments and medications as far as their benefit to the individual, and makes reports to the medical team.
+ Determines the best way (within medical orders) to administer the medication and in what order.
+ Responsible for determining the comprehensive needs/desires of individuals with intellectual/developmental disabilities.
+ Makes decisions about how to ensure individuals’ rights are respected and protected.
+ Responsible for determining how to encourage individuals to become more independent in their personal lives, ensuring they are safe and healthy.
+ Makes decisions about individuals’ abilities and documents findings on the residential assessment to be used in formulating the IPP.
+ Determines if individuals can be safely transported to and from the home, depending upon the weather and the individuals’ health.
+ Responsible for determining how to complete annual Money (CIC) Assessments, Comprehensive Life Reviews, and updating Cues and Clues for individuals' Individual Program Plan (IPP);
+ Determines how to provide ongoing formal and informal Active Treatment training to direct care staff;
+ Decides base lining, writing, and implementing money management learning objectives based on the annual CIC assessment; and
+ Determines how to update the Active Treatment (ATX) guidebook and completes ATX/Case Record book audits monthly to ensure continuity of care across all disciplines’ learning objectives and service plans.
+ Decisions regarding the completion of the above tasks are made independently.
+ Responsible for determining and creating monthly activity calendars and ensuring all individuals have at least 4 meaningful community participation activities each month.
+ Uses person-centered planning to determine plans and implement meaningful community outings based on individuals’ preferences and abilities.
+ Determines how to ensure individuals have weekly spending money and/or group activities money, and to ensure direct care staff have the required resources to take individuals on meaningful community outings.
+ Participates in pre-admission meetings for new individuals, monthly team meetings, annual IPP meetings, and Interdisciplinary Team Meetings (IDT), and determines what feedback to provide as it relates to money management, ATX, and community participation.
+ Make decisions related to specific money management goals for individuals, what ongoing community outings they want to participate in, and what formal and informal active treatment they will benefit most from.
+ Other duties as required for business needs.

+ High school diploma or General Education Diploma (GED)
+ Three (3) years of experience in an occupational field related to the work assigned to the position
+ Certification or licensure appropriate to the work assigned to the position
Substitutions
Additional appropriate education will substitute for the required experience on a year-for-year basis.

+ Out-of-State Driver’s License holders must obtain a valid, non-restricted Colorado Driver’s License within 30 days of employment start date.

+ Pre-employment Physical/Test: Must demonstrate the ability to lift 50 lbs, bend, squat, etc., and perform physical interventions proficiently (pre-employment and continuously throughout employment).

+ A Tuberculosis Test (TB) will be required upon hire
+ WRRC is a 24/7 facility; therefore person in this position must be available to work any shift, any day, and/or any area, to meet agency needs.
+ Must travel to remote work sites and must utilize a state vehicle, if available. If not, must utilize personal vehicle and submit for mileage reimbursement. Position is required to use a state vehicle to transport residents.

CDHS Selection Process Explanation
Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a prorated basis to determine qualifications.
