    BAYADA Home Health Care is seeking a **RN** **Case Manager f** or our **West Chester, PA Pediatrics and Adults home care office** . This office provides best in class care to clients throughout West Chester, PA and surrounding areas.

    Responsibilities for a RN Case Manager:

    + Lead, inspire and motivate others to provide exceptional care.

    + Visit our Pediatric/Adult clients and conduct assessments in West Chester, PA and other surrounding areas.

    + Develop the care plan and oversee its execution.

    + Supervise LPN's, RN's, and home health aides (HHA) staff in the field.

    + Facilitate new hire orientation.

    + Conduct competencies on new and tenured staff.

    Qualifications for a RN Case Manager:

    + Registered Nurse (RN) with 1 or more years of experience as an RN.

    + Supervisory experience preferred.
