    Description

    **Job Purpose:** This is a professional position that is responsible for the care and treatment of adults who struggle with mental illness and/or substance use. CCS is a community-based program in which the majority of services are provided in clients’ homes and in the communities. The Lead Care Coordinator will train new staff, audit case notes and clients’ electronic health records, and perform other various administrative tasks assigned by the CCS Administrator. This role will also act as a Care Coordinator who meets and works with the clients, service providers, and other team members who assist in the client’s progression. This role will develop and implement treatment plans, follow-up, case management, and intervention as required.

    + Lead and model the Comprehensive Community Services process.

    + Audit case notes, treatment plans, program enrollment, and tracking of documentation.

    + Train, onboard, guide employees, provide ongoing support, and leadership to the entire team.

    + Complete at least eighty percent of training for new staff, the remainder will be shadowing and delegating to various other CCS Care Coordinators.

    + Provide hands-on training in the community with all new Care Coordinators.

    + Attend team meetings for new hires as needed to provide coaching and maintain compliance with policies.

    + Engage in problem-solving and feedback to Care Coordinators.

    + Record meeting minutes and share with members of the team.

    + Ensure note templates remain current by incorporating Milwaukee County feedback and regulations.

    + Participate in quality improvement initiatives related to care coordination outcomes.

    + Collaborate with leadership to develop policies, workflows, and training programs.

    + Integrate and promote CCS into internal programs in CFI to increase volumes of customers serviced.

    + Serve as a back-up Intake Coordinator as needed.

    + Develop a treatment relationship with clients and formulate individualized treatment plans.

    + Provide client transportation in a personal vehicle as needed.

    + Provide supportive community visits to clients’ homes.

    + Establish client’s eligibility by using a Mental Health/AODA Functional Screen.

    + Assist the client by identifying CCS service array appropriate to address recovery needs.

    + Responsible for maintaining treatment-oriented environment (under supervision of direct supervisor) and assure consistency in carrying out treatment objectives.

    + Broker services as needed with other providers in the CCS network and follow up to ensure quality of care and goal progression.

    + Facilitate recovery meetings with the client and the team.

    + Responsible for formulation of appropriate discharge plan with assistance of client and direct supervisor.

    + Provision of ongoing assessment of clients demonstrated in case note submission, biannual assessments, maintaining case files as required by agency policy, and applicable local, state and federal codes.

    + Meet department-designated monthly billing quotas, EHR note entry within 24 hours, biannual treatment plans and other funder required documentation.

    + Attend and participate in staff meetings, in-service training, and conferences as required.

    + Providing client coverage for other staff members as assigned.

    + Adheres to agency and Milwaukee County (funder) policies and procedures while carrying out daily responsibilities.

    + Attendance at community-based MC3 Change Agent meetings as delegated.

    + Required participation in recurring on-call duties and flexibility in daily work schedule to adequately meet program needs, which may include evenings or weekend.

    + Required attendance in Trauma Informed Care Trainings.

    + Other duties as assigned.

    Qualifications

    **Required Education, Experience, Certifications, Licensure and Credentials** : (Where appropriate, education and/or experience may be substituted)

    **Minimum Required Education** : Bachelor’s degree in the field of Social Work, Counseling, or related Human Services Field. Master’s preferred.

    **Minimum Required Experience** : Experience working with adults with severe and persistent mental illness and/or substance use disorders. Also have 2 years’ experience working in the CCS Program.

    **Required License – Certification – Registration** : None

    **Travel Type** : Road Warrior

    **Required Valid Driver’s License** : Valid driver’s license

    **Required Auto Insurance** : Vehicle liability insurance in accordance with agency policy.

    Knowledge, Skills, & Abilities:

    + Proficient in Microsoft Office, such as Word, Outlook, Teams, and basic office technology (e.g., computer, cell phone, fax).

    + Ability to maintain good communication and promote positive working relationships and work openly and effectively with one’s direct supervisor, colleagues, and outside agencies.

    + Build and maintain client satisfaction with the services offered by the organization.

    + Diligently attends to details and quality in accomplishing tasks in a fast-paced work environment.

    + Certification as a functional screener and experience with motivational interviewing are preferred.

    + An understanding of mental health, substance use, and co-occurring needs and the ability to clearly communicate these to others.

    + Ability to support emergency situations.

    + Knowledge of Milwaukee County TCM, CCS, and/or CSP/ACT Case Management is preferred.

    + Reliability in reporting to work regularly and on time.

    + Access to a personal vehicle.

    Physical Requirements, Visual Acuity, and Work Conditions:

    **Physical Requirements** : On occasion, must be able to push/pull/lift/move objects weighing at least 20 pounds. Must have the ability to climb at least two flights of stairs. Work is performed largely in the community but may at times include an office environment or other various settings.

    **Visual Acuity** : The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.

    **Working Conditions** : This is a community-based position. Staff are expected to utilize their personal vehicles to transport clients and meet with clients in their homes. This position may work in a variety of indoor and outdoor work environments which may include various outdoor weather conditions, loud noises, fluctuating temperatures and the occasional use of stairs. The hours of work will generally be during regular business hours and average at least 40 hours a week. This position requires participation in recurring ‘on-call’ duties and flexibility in daily work schedule to adequately meet program needs, which may include evenings or weekends.
