    Field Nurse (RN) – Direct Hire Opportunity

     

    Adecco Healthcare & Life Sciences is proud to partner with a leading home healthcare organization to hire an experienced and compassionate **Field Nurse** (Registered Nurse) for a direct hire opportunity.

     

    The **Field Nurse** will provide comprehensive patient care in the home setting, ensuring quality outcomes and strong communication across interdisciplinary teams. This is an excellent opportunity for an RN who thrives in independent, patient-centered work and values a collaborative care model.

     

    **Salary Range** : $105,000 – $110,000/year

    **Location:** Bronx, NY

    **Employment Type:** Full-Time, Direct Hire

    Key Responsibilities

    • Conduct home visits and manage all aspects of patient care in accordance with physician orders and agency policy

    • Complete all required documentation accurately and promptly in HomeCare HomeBase (HCHB)

    • Communicate effectively with physicians (PCPs), interdisciplinary teams, and supervisors

    • Prepare and update Plan of Care (485) and all interim orders

    • Maintain and update the medication profile and coordinate care across disciplines

    • Review and complete case communication notes, follow-up items, and any incomplete documentation

    • Participate actively in case conferencing and care coordination meetings

    Qualifications

    • Active RN license (state-specific) in good standing

    • 2+ years of clinical nursing experience, preferably in home health or community-based care

    • Strong organizational, documentation, and communication skills

    • Proficient with electronic medical record systems (HCHB experience preferred)

    • Ability to manage multiple priorities and work independently in the field

    $15,000 Sign-On Bonus (paid after 15 months and full productivity)

    **Pay Details:** $105,000.00 to $110,000.00 per year
