    Position Summary

    This is advanced level work. At this level, employees independently perform, complex and difficult reviews and determinations with limited supervision. This work requires applying a logical or scientific understanding, skill in analyzing and interpreting data to formulate recommendations of a specialized nature for eligibility for APD services. FAC governs the timelines and other requirements for this position. In all circumstances, the employee must be in compliance with the timelines and requirements outlined in the FAC.

     

    The Work You Will Do

     

    Reviews case documentation and supporting medical, therapy and school evaluations for the purpose of eligibility determination Interviews individuals, or families, and reviews records to obtain information required for eligibility determination. Analyzes information to assess the applicant’s developmental disability; may refer the applicant to other specialists, institutions, or support services. Schedules additional testing/evaluations with community Florida licensed Psychologists, Psychiatrists, Pediatric Neurologists and/or other accepted professionals per FAC. Reviews for High Risk designation: psychosocial risk assessments, evaluations and developmental screening of high risk individuals and/or families. Communicates with families and participates in staffing’s with other agencies. Communicates with co-workers, agencies, supervisors, and others regarding eligibility determinations as appropriate and with the proper consents. Tracks timelines and assures compliance with the requirements and within the timelines in the FAC.

     

    Serves as the agency representative when individuals are determined to be ineligible and an appeal hearing is requested. Works with the Region’s attorney to get prepared for hearings and testifies accordingly. Provides information requested by the attorney. Prepares the evidence packet for submission to the attorney. Prepares Legal Notices for both those who are determined to be ineligible or eligible, mails Notices to applicants.

     

    Provides information regarding the interpretation of psychosocial assessment data of individual clients. Support ineligible determinations using accepted definitions/criteria for covered diagnosis per Rule/Statute. Exchanges information with other health and/or rehabilitative agencies to provide continuity of care for clients receiving services from more than one agency.

     

    Makes presentations to agency and community groups regarding APD services, resources, and community service access. Knowledge regarding community programs which provide intervention strategies necessary to increase social support services for at risk clients in the community. (ie local Mental Health programs, ARC etc).

     

    Assigned applicants are entered into the iConnect system and all required screens are updated to reflect the most current information and APD status of the individual. All cases received from Intake for eligibility determination are reviewed and determination of eligibility is made within the 45-90-day time frame of signed application. Act in accordance with requests for assistance with occasional miscellaneous tasks: Participate in Hearings, Lockout calls, attend to walk-ins, attend community events, respond to Suncoast Region emails, collateral requests for information, follow ups with applicants, families, schools, and medical offices etc. Other duties as assigned.

    Minimum Qualifications

    + High school diploma or its equivalent.

    + Must have valid Driver’s Licenses or other efficient means of transportation to travel for work purposes.

    + Must have case management experience.

    + Must have customer service experience.

     

    Knowledge, Skills, And Abilities

    Knowledge of:

    + Florida Statute 393.65 preferred.

    + Rule F.A.C. 65G-4.01, 4.015, 4.016 and 4.017.

    + Using databases, electronic files, and Microsoft Office Programs.

    Ability to:

    + Plan, organize and coordinate work assignments.

    + Communicate effectively verbally and in writing with clients and other agencies.

    + Establish and maintain effective working relationships with others.

    + Manage a complex case load.

    + Represent the programs, policies, and procedures in an assertive and professional manner.

    Direct Deposit Program

     

    As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program. Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. Retirement funds are also required to be in the Direct Deposit Program.

     

    Background Screening Requirement

     

    It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.
