    Job Summary and Qualifications

     

    The Discharge Planner and Case Coordinator is responsible for coordinating care and discharge of behavioral health patients for the Senior Behavioral Center at Memorial Satilla Health and provide support and education to patients and families relevant to inpatient care and continuation of services for discharge.

     

    The Discharge Planner and Care Coordinator works with appropriate parties and physicians through the patient response inquiry, admission, treatment and discharge process. D/C Coordinator will work collaboratively with members of the health care team, demonstrates competency to provide age appropriate care to geriatric patients, and provides patient and family education. Discharge/Care Coordinator provides professional therapeutic services to behavioral health patients and participates in multidisciplinary treatment team plan of care.

    Major Responsibilities:

    Accountabilities

    •

     

    Coordinates and participates with appropriate interested parties and physicians through the patient response inquiry, admission, treatment and discharge process.

     

    •

     

    Completes assessments in a timely manner–90 minutes or less on average.

     

    •

     

    Provides behavioral health education to patients and families as needed.

     

    •

     

    Provides resources and support for patient and families.

     

    •

     

    Models and trains other behavioral health team members on managing and tracking inquiry calls.

     

    •

     

    Tracks and reports on all key indicators related to calls, assessment, admissions.

     

    •

     

    Participates and/or supports HBIPS and other quality indicators.

     

    •

     

    Ensures patient rights and confidentiality are maintained.

     

    •

     

    Provides input, suggestions and support appropriately.

     

    •

     

    Participates as an active member of the multidisciplinary treatment team.

     

    •

     

    May be required to complete preauthorization of inpatient psychiatric treatment.

     

    •

     

    Works collaboratively with hospital departments to assist managing behavioral health level of care determination clinical assessments.

     

    •

     

    Conduct discharge group meetings, providing supportive resources, discuss transition, identify potential risks or barriers to success, and providing Q&A to patients at regular intervals throughout care.

     

    •

     

    Conduct psychoeducational group sessions, process groups, support groups, and individual/family meetings as needed or designated.

     

    •

     

    Completes additional duties as assigned.

    Provides supports by:

    Promoting consistent positive patient interactions that advance the agenda of unparalleled patient service

     

    •

     

    Practicing and adhering to the “Code of Conduct” philosophy and “Mission and Value Statement”

     

    •

     

    Performing other duties as assigned

    Education & Experience:

    •

     

    Master's degree in Social Work, LCSW, Counseling or related field with experience as outlined below 1+ year(s) of experience in Case management, discharge planning, group leadership skills, age specific growth and development and crisis and behavioral management.1+ year(s) of experience in Behavioral healthcare setting, performing clinical assessments or in a clinical treatment setting for non-nurses.
