Licensed Practical Nurse
ESSENTIAL DUTIES AND RESPONSIBILITIES (include the following. Other duties may be assigned.)
+ Provide nursing care and treatment to individuals as prescribed by licensed providers and as directed by the supervising RN. This may include repositioning, mealtime assistance, and personal care.
+ Administer medications and treatments in accordance with provider orders, OPWDD guidelines, and agency policy.
+ Observe, record, and report changes in individuals' physical or mental health status to the RN or provider promptly.
+ Assist in the implementation of each individual's Plan of Nursing Services (PONS) and contribute to updates through observation and documentation.
+ Maintain accurate, timely, and complete health and medication records in compliance with OPWDD and agency standards.
+ Participate in the prevention, identification, and management of illness or injury.
+ Work collaboratively with the RN's, clinicians, and interdisciplinary team members to ensure continuity of care.
+ Communicate effectively with residential and day program staff regarding individuals' health needs and any changes in condition.
+ Support and educate direct support staff in the implementation of health-related protocols and infection control practices.
+ Ensure medication orders are current, properly transcribed, and stored securely.
+ Report medication errors or variances to the RN immediately and assist in corrective actions as directed.
+ Support the RN in monitoring AMAP-certified staff for adherence to medication administration procedures.
+ Accompany individuals on medical appointments or to the hospital/ED as requested.
+ Complete required agency, state, and federal documentation promptly and accurately.
+ Assist with audits, reviews, and surveys as requested by the RN or management.
+ Participate in training and competency reviews required by the agency and OPWDD
+ Reinforce teaching provided by the RN or medical providers related to diet, hygiene, safety, and medication adherence.
+ Implement infection control procedures and participate in health and safety monitoring.
+ Respond appropriately to emergencies, providing first aid and contacting medical professionals as necessary.
+ Promote a safe, respectful, and inclusive environment that supports individual choice and independence.
+ Practice within the legal scope of an LPN as defined by the NYS Nurse Practice Act and under direction of an RN.
+ Maintain confidentiality and comply with HIPAA and OPWDD standards.
+ Demonstrate professionalism, accountability, and ethical nursing practice.
+ Participate in ongoing professional development and maintain an active NYS LPN license.

QUALIFICATIONS

EDUCATION AND/OR EXPERIENCE
Current licensure in New York as an LPN and a valid New York State driver's license. One year's experience preferred, experience with individuals with developmental disabilities preferred.
LANGUAGE SKILLS
Ability to read and interpret documents such as safety rules, operating and maintaining instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively to groups of customers or employees of an organization. Strong communication skills.
MATHMATICAL SKILLS
Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instruction furnished in written, oral, diagram, or schedule form.
COMPUTER SKILLS
Must be knowledgeable about Internet software, Microsoft Office components including Outlook. Becomes knowledgeable and trained in the use of Therap for plan writing, event reporting and ongoing communication.
CERTIFICATIONS, LICENSES, REGISTRATIONS
Must possess a valid NYS driver's license.
PHYSICAL DEMANDS
+ Ability to stand, walk, bend, reach, and lift up to 50 pounds as part of normal job duties.
+ Must be able to assist in lifting or transferring individuals with proper equipment or team support.
+ Visual and auditory acuity sufficient to assess health conditions and safely administer medications.
+ Manual dexterity sufficient for safe medication administration and documentation.
+ Ability to respond quickly and effectively to medical or behavioral emergencies.
WORK ENVIRONMENT
This position operates in OPWDD-certified residential and day program settings and may involve exposure to communicable diseases, behavioral challenges, and varying environmental conditions. The LPN may be required to travel between sites and participate in on-call or weekend coverage based on program needs. Work hours may include evenings, holidays, and emergencies.
The rate of pay for this position is $55,120 annually, based on $26.50/hour
