• RN Case Manager

    Amergis (Fairfield, CA)


    The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

    Minimum Requirements:

    + Current RN licensure in state practicing

    + At least one year of Case Management experience preferred

    + Current CPR if applicable

    + TB questionnaire, PPD or chest x-ray if applicable

    + Current Health certificate (per contract or state regulation)

    + Must meet all federal, state and local requirements

    + Must be at least 18 years of age

     
     
     
     

    + $2,240 taxable income

    + $1,246 in stipends

     
     

    Join a dynamic healthcare team in Fairfield, CA as a Travel RN Case Manager! In this role, you’ll assess, plan, implement, monitor, and evaluate individualized care plans for hospital patients across the continuum of care. You’ll collaborate with interdisciplinary teams and social services to ensure optimal patient outcomes while balancing clinical needs with financial and health plan resources.

     

    This position requires strong clinical judgment, communication, organization, and time management skills. You’ll be working with patients experiencing a range of conditions including depression, psychosis, substance use, and other significant mental health challenges.

     
     

    + Schedule: Monday–Friday, 8:00am–5:00pm

    + Some weekend coverage required (mostly Saturdays)

    + Duration: 8-week initial contract

    + Setting: On-site only (no remote work)

    ? Requirements:

    + Active CA RN License

    + Minimum 2 years of Case Management experience

    + Experience with EPIC EMR

    + BLS Certification
