    AdventHealth Senior Care is looking for a Social Worker Team Lead to be responsible for social work practice and supporting the Social Work Manager/Senior Manager/Director of Suppor and Services. The primary responsibilities of the Social Worker center around problem-solving, providing resources, services and opportunities to enhance coping capacities for members. The Social Worker serves as the subject matter expert for the members psychosocial and behavioral health needs. The Social Worker also contributes to the development and improvement of social policy in the primary care setting and community.

     

    The Social Worker Team Leader under the direct supervision of the Well 65 Manager/Senior Manager is responsible for the support, education, competency review and continuous training of Social Workers. Leadership and relationship building skills, customer service and a commitment to team-based healthcare are essential in all aspects of this position. The Social Worker is an important part of AdventHealth Senior Care and supports the care team in decision-making to provide a high-quality care experience for members.

    The value you will bring to the team:

    Provides managerial guidance and support to Social Workers under the direct supervision of the Social Work Manager/Senior Manager.

     

    Oversee sustained progress of social work programs, standardization of care protocols, and training.

     

    Monitor analytical reports and take appropriate action to ensure standards of care and performance are met thru EMR dashboards and chart reviews.

     

    Under the Social Work Manager/Senior Manager provides training to new hires and assistance annual performance/competency evaluations.

     

    Under the supervision of the Social Work Manager/Senior Manager provides development education and training pertaining to social work department and role.

     

    Function as a treatment team member, interpreting social, psychological, emotional, and family system problems and strengths to other members of the treatment team.

     

    Provide onsite/remote coverage to clinics for Social Work needs during clinic vacancy, PTO and absences.

     

    Incorporates patient, physician, customer needs and concerns into decision-making.

     

    Demonstrate ability to provide social work services to members, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge.

     

    Initiate a Baker Act.

     

    Identify and intervene for suspected abuse, including sensitivity to age specific concerns: physical/emotional/sexual abuse/domestic abuse; caretaker/institutional abuse for senior adults.

     

    Manage crisis situations as needed, demonstrating ability to quickly appraise crisis and select appropriate intervention.

     

    Document direct service in members medical record after each significant encounter.

     

    Demonstrate proficiency in use of the electronic medical record.

     

    Attend and participate constructively in meetings with team members, practice manager, provider, and all other meetings, both on and off site as assigned.

     

    Provides coverage and Managerial/Senior Managerial duties as assigned.

     

    Communicates effectively and respectfully with members, vendors, and all levels of team members and management.

     

    Provides conscientious commitment to assigned work, work schedule, honesty, teamwork, assisting others to bring about positive outcomes.

     

    Recognizes and manages risks, resolves issues and problems as soon as they arise, reports and documents adverse events and reportable conditions.

     

    Adhere to policies, procedures, protocols, and principles established by Senior Care and other accreditation and/or regulatory bodies.

     

    Accept and thoroughly and skillfully complete special projects as assigned.

     

    Clinical Competency Reviews thru ongoing chart audits of Social Work Team Members.

     

    Monitor analytical reports and take appropriate action to ensure standards of care and performance are met thru EMR dashboards and chart reviews.

     

    Under the Social Work Manager/Senior Manager/Director of Support and Services provides training to new hires and assistance annual performance evaluations.

     

    Under the supervision of the Social Work Manager/Senior Manager/Director of Support and Services provides development education and training pertaining to social work department and role.

    The expertise and experiences you'll need to succeed:

    **Minimum qualifications** :

    Masters degree

    3 years experience

     

    Maintains Florida State (Chapter 491 Florida Statues) licensure as a Licensed Clinical Social Worker
