    Job Summary

Join our dynamic team as an Eligibility Representative in the Connecting Seniors to Coverage program, a vital part of Tufts Health Plan Senior Care Options (SCO). In this impactful role, you will assist seniors aged 64+ who are presumed eligible for MassHealth Standard, Medicare, and/or the Home and Community-Based Waiver. Your responsibilities will include educating seniors and their caregivers on eligibility guidelines and benefits, guiding them through the application process, and driving SCO membership growth. You will work closely with seniors identified by the SCO Team, referrals from other Point32Health departments, external brokers, and through your own efforts to generate new leads.

This role requires meeting monthly MassHealth enrollment goals and the ability to work independently in a fast-paced environment while serving a culturally diverse population. An in-depth knowledge of Medicaid and Medicare policies and waiver regulations are crucial to the success of this role. Key components of this position include engaging directly with prospects and members, managing a high volume of inquiries and applications, serving as the primary resource and focal point for MassHealth and Medicare enrollment support across the organization, overseeing outreach activities, and fostering community partnerships. Strong multitasking, keen attention to detail, anticipation of needs, and problem-solving abilities are crucial. Additionally, you will play a vital role in educating internal staff and external partners, such as Aging Services Access Points (ASAPs), ensuring everyone is well-informed about the Connecting Seniors to Coverage program and the Senior Care Options (SCO), and aligned with our mission.
