Job Summary:
*This is a hybrid position with required in-person responsibilities at the Northshore YMCA and in the Bothell community.
The YMCA of Greater Seattle is a contracted partner to provide case management services across King County as part of HealthierHere’s Community Hub. YGS is also partnering with the City of Bothell through our new Northshore YMCA Health Navigation Hub. We are hiring for a Community Hub Navigator who will perform case management duties, both in-person in Bothell and virtually, and provide program coordination/administration support directly in our Northshore YMCA Health Hub. The Community Hub Navigator will provide resource navigation to support individuals in identifying and achieving their goals related to whole person health and support administrative functions of the Northshore YMCA Health Hub. Functions of this position may include but are not limited to: following best practices in assessing and addressing Health-Related Social Needs (HRSN) of individuals; connecting clients to appropriate internal and external resources to address health and social needs; building strong relationships with internal and external partners, including YMCA staff and clients; conducting outreach to increase awareness of available community services and build local partnerships; and triaging incoming referrals to Y services housed within the Northshore Hub. The Navigator will carry a caseload of clients, and the number may vary based on assignments by HealthierHere and Bothell residents seeking services. Client interactions will be in-person at the Northshore YMCA Branch and virtually through phone calls, text messages, and emails. Some in-person training and meetings will also be part of this role.
Hiring Range: $25 - $28/hour

Responsibilities
+ Maintain and support a caseload of clients with complex social and health-related needs across King County
+ Provide timely outreach, intake, enrollment, and assessment to all assigned clients
+ Develop individualized goals and service plans to assist referred clients in connecting to internal and external resources related to social services (housing, employment, food access, etc.), healthy lifestyles, primary medical and behavioral health care, and other services as assigned
+ Based on knowledge of the Social Work field and profession, follow best practices for YMCA Healthy Living department approaches to addressing Health-Related Social Needs, including efficiently and successfully connecting clients to services in the community
+ Help clients navigate the social service and healthcare landscape in King County, including those with complex needs related to housing, substance use support, behavioral health, and more
+ Provide information and education to clients to help them become more confident in accessing resources and be an active participant in their care coordination
+ Ensure program outcome requirements are met and services are rendered appropriately by understanding contract/grant terms and conditions
+ Utilize best practice approaches in the field, including trauma-informed care, motivational interviewing, social service navigation, etc.
+ Maintain clear, professional, and timely participant contact notes, service plans, progress updates, outcomes, and other documentation in designated databases
+ Triage incoming referrals for the Northshore Health Navigation Hub and direct clients to available Y services and programs as appropriate
+ Collaborate with other YMCA staff and/or community partners to plan and facilitate activities, including visits to partner organizations in the community
+ Build and maintain collaborative relationships with funding liaisons, other service providers, and other community organizations as assigned
+ Complete required trainings as directed in established timeframes
+ Attend outreach events such as resource fairs and community gatherings to promote services available to individuals
+ Other duties as assigned

WORK ENVIRONMENT
This position will include a combination of office, remote, and field work. This position requires in-person work at the Northshore YMCA branch.
PHYSICAL DEMANDS

While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. This position requires the ability to occasionally lift office products and supplies, up to 20 pounds.
POSITION TYPE/EXPECTED HOURS OF WORK
This is a full-time position working a minimum of 30 hours and a maximum of 40 per week, based on contract need, based at the Northshore YMCA branch. Evening and weekend work may be required as job duties demand.
TRAVEL
Travel is primarily local, although some out-of-the-area and overnight travel may be expected.
Qualifications
+ Bachelor’s degree in social work or related field and two years of paid social services experience; or Bachelor’s degree in unrelated field and four years of paid social services experience; Or Associate’s degree in other major with six years of experience working in social work or human services may be considered
+ Intermediate computer skills and experience with Microsoft Office suite.
+ Strong written and oral communication skills
+ Strong organizational skills with attention to detail and the ability to manage multiple tasks and projects
+ Ability to visit sites and community organizations throughout King County, including Bothell and surrounding area
+ Knowledge of and previous experience with diverse populations (language, culture, race, physical ability, sexual orientation, etc.)
+ Experience working collaboratively with multiple stakeholders, such as customers, community members and organizations, businesses, and co-workers.
Preferred Qualifications:
+ Experience supporting clients in addressing complex legal, housing, and mental and behavioral health-related needs
+ Familiarity with resources and social services available in King County
+ Experience in customer service-related roles
+ Ability to speak any language in addition to English may be helpful
+ Proven history of successfully following project expectations on time and within budget
