    Speech-Language Pathologist (Per Diem) – In-House Subacute Rehabilitation

    ? Location: Pine Acres Health & Rehabilitation Center – West Des Moines, IA

    About the Role:

    Pine Acres Health & Rehabilitation Center is seeking a dedicated and compassionate Per Diem Speech-Language Pathologist (SLP) to join our in-house subacute rehabilitation team. This position offers the opportunity to work in a supportive, patient-focused environment where you can make a lasting difference in the lives of residents recovering from illness, injury, or surgery.

    Key Responsibilities:

    + Evaluate, diagnose, and treat communication, cognitive, and swallowing disorders in adult and geriatric populations.

    + Develop and implement individualized treatment plans to maximize functional outcomes.

    + Collaborate closely with an interdisciplinary team, including physical and occupational therapists, nursing, and medical staff.

    + Provide education and training to patients, families, and caregivers regarding therapy techniques and progress.

    + Maintain accurate, timely, and compliant documentation within facility and regulatory standards.

    Qualifications:

    + Master’s degree in Speech-Language Pathology from an accredited program.

    + Current Iowa SLP license (or eligibility).

    + Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; CFY considered.

    + Prior experience in skilled nursing, subacute, or long-term care setting preferred.

    + Strong interpersonal and communication skills.
