Job Description

The Iowa Department of Health and Human Services (HHS), Division of Aging and Disability Services, Bureau of Community Integration, Targeted Case Management (TCM) Unit is seeking a Social Worker 6 to join our team!
Under the general supervision of the Bureau Chief of Community Integration, this position serves as a statewide resource for community integration, high level case review, and consultation and guidance on critical, complex, and high needs cases for individuals served by the HHS TCM unit or other partners in the Medicaid Fee For Service (FFS) system. This position will have the responsibility of providing technical assistance and making recommendations regarding development or revisions to policy and procedures consistent with the application of Iowa Code, Administrative Code, and HHS policies.
This position also collects, tracks, analyzes and interprets data for ongoing reports related but not limited to the health and safety outcomes of the individuals served. This position will create reports including annual reports and summary tracking reports to demonstrate the efficacy of high-level case review. The SW6 will meet with internal and external stakeholders and represent the Department on various committees and workgroups. Responsibilities may include responding to requests for information from the legislature, stakeholders, and the public; and assisting with providing data and analysis on services provided by the unit
Participates in and works with the HHS TCM Training and Support Team regarding the development and delivery of training, including the development of training materials related to new and/or revised policies and procedures for staff. Provides training, technical assistance and consultation to agencies/entities engaged in community integration efforts for individuals with disabilities.

+ Respond quickly to information inquiries from the government, various state entities, and the public.
+ Multi-task and utilize time management skills to meet activity demands by prioritizing multiple requests for consultation and policy clarification in conjunction with miscellaneous project deadlines.
+ Interact and communicate effectively with a variety of customers from different backgrounds in person, over the phone, and in writing.
+ Possess computer skills and use a variety of software applications and products, including Microsoft Office.
+ Communicate effectively and accurately by telephone and in person as well as deal appropriately and courteously with customers and other community partners.
+ Read and accurately interpret federal statute and regulations, state statute and rules and existing policy.
+ Work on detailed projects with consistent results and minimal "hands-on" supervision showing commitment, effort and initiative to timely project completion.
+ Display high standards of ethical conduct, including maintaining confidentiality.
+ Work as a part of a team to provide consulting services; identify, develop, and deliver staff training as needed; and accomplish service program goals, monitor progress, and evaluate outcomes.
Current experience in the policies and practices of HHS Targeted Case Management is highly desired.
Applicants must possess 1 years’ experience in delivery in one or in a combination of the following areas of human services:
+ Chronic mental illness
+ Developmental disabilities
+ Intellectual disabilities
OR
Applicants must have an Iowa License to practice as a registered nurse AND the equivalent of 3 years nursing or human service experience.

Minimum Qualification Requirements
Graduation from an accredited four year college or university and the equivalent of six years of full-time experience in a social work capacity in a public or private agency;
OR
a Bachelor's degree in social work from an accredited four year college or university and the equivalent of five years of full-time experience in a social work capacity in a public or private agency;
OR
a Master's degree in social work from an accredited college or university and the equivalent of three years of full-time experience in a social work capacity in a public or private agency;
OR
an equivalent combination of graduate education in the social or behavioral sciences from an accredited college or university and qualifying experience up to a maximum of thirty semester hours for one year of the required experience;
OR
employees withcurrentcontinuous experience having earned their equivalent full-time social work experience with the Iowa Department of Human Services may qualify with one year less of the required experience.
