Job Summary and Qualifications
Responsible for facilitating the Patient Safety Program with the goal of protecting patients from harm, and developing strategies to prevent future harm through process improvement and staff education.
Responsible for overseeing patient safety activities, which include, but may not be limited to managing and analyzing close calls and adverse patient events, conducting educational programs, leading clinical initiatives, and complying with regulatory standards with the objective of enhancing and promoting patient safety and quality care.
Participate in the clinical risk management program, including the functions of clinical risk management, claims management, program administration and risk management education.
Patient Safety
Actively collaborates in the annual review/revision of the organization’s Patient Safety Program Plan
Maintains current knowledge of trends and initiatives related to patient safety
Supports patient safety initiatives through leadership, direction and active involvement
Acts as resource, internal consultant, and educator for patient safety issues
Assists in the development of evidence based practice to comply with TJC and NPSF Patient
Safety Initiatives
Develops and implements programs designed to minimize the frequency and reduce the severity of actual and potential patient safety hazards throughout the facility
Participates in patient safety activities with other clinical disciplines—Medical Staff, Clinical Staff, Quality Management, Infection Prevention, and Risk Management
Actively participates in SEAs (Serious Event Analysis), and recommendations for improvement
Actively participates in FMEA (Failure Mode and Event Analysis)
Leads investigations for serious adverse events
Participates on Hospital Committees, as assigned
Event Reporting
Meets state-defined requirements for event reporting
Provides data analysis, trending and recommendations to various patient safety and performance improvement committees
Conducts systems analyses to uncover and identify trends that could potentially result in serious events
Operations/Compliance
Actively collaborates in the annual review/revision of the organization’s Patient Safety Program Plan, with responsibility for operationalizing the program
Ensures patient safety by following-up and acting on all regulatory recommendations/deficiencies
Identifies and communicates regulatory requirements related to patient safety activities.
Assists in the development of departmental and facility policies and procedures that affect patient safety and regulatory compliance
Plans, develops, and presents educational material to administration, medical staff, nursing personnel, and other departmental personnel on topics related to patient safety
Clinical Risk Management
Assists in the development of Hospital Risk Management Program
Assists with the annual evaluation of the effectiveness of the Risk Management Program and develops/modifies goals as indicated
Assists with the claims administration and management of compensable / potentially compensable events including PCRs, and investigations.
Education & Experience:
Required: 4 years clinical experience in a hospital or healthcare setting
Preferred: Certified Professional in Patient Safety (CPPS) preferred
Required: Baccalaureate degree in a health care field. Registered Nurse license in state of Virginia.
