    Licensed Practical Nurse (LPN) – Skilled Nursing Facility

     

    We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide exceptional care and contribute to a supportive and professional environment.

    Responsibilities:

    + Deliver direct nursing care to residents in accordance with the care plan

    + Administer medications and treatments as prescribed

    + Monitor residents' conditions and report changes to the Registered Nurse or physician

    + Document resident care accurately and timely

    + Supervise Certified Nursing Assistants (CNAs) and ensure care plans are followed

    + Assist with admissions, transfers, and discharges

    + Communicate effectively with residents, families, and interdisciplinary team members

    + Maintain infection control and safety standards in accordance with facility policies

    Qualifications:

    + Active LPN license
