    Responsibilities

    Educate patients, healthcare workers and the community about controlling diabetes through nutritional management.

     

    + Coordinate with designated staff to effectively integrate medical nutrition therapy into the overall management of diabetes.

    + Provide diabetes education for patients, healthcare workers and the community.

    + Encourage the diabetes team to support and reinforce nutrition prescriptions and to help patients manage their meal plans.

    + Supervise the planned education process for the goals of medical nutrition therapy for diabetes.

    + Reinforce diabetes education and instruction to the staff and implement and monitor educational plan of care.

    + Coordinate with healthcare providers including receiving and writing orders to ensure effective and efficient patient care and achievement of desired patient outcomes.

    Requirements

    Licensure / Certification / Registration:

    + One or more of the following required:

    + Dietitian/Nutritionist credentialed from the Commission on Dietetic Registration obtained prior to hire date or job transfer date.

    + Dietitian/Nutritionist credentialed from the Illinois Department of Financial and ProfessionalRegulation obtained prior to hire date or job transfer date.

    + Educator specializing in Diabetes Management preferred.

    Education:

    + High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.

    Additional Preferences

    + Prior high-risk obstetrics patient experience.

    + Prior diabetic education experience.
