    JOB DESCRIPTION

    Job Summary:

    The Transition Specialist / Work - Based Learning Coordinator is responsible for planning , implementing , and overseeing transition services and work - based learning ( WBL ) opportunities for students with disabilities ( and / or those needing transition supports ) to ensure successful post - school outcomes in alignment with federal and state regulations .

    This includes :

    + coordinating all phases of the work - based learning process

    + tracking student WBL hours

    + handling referrals and application processes to agencies such as ACCES - VR

    + developing employer partnerships

    + preparing students in employability skills

    + Collaborating with teachers for students aged 14+ to support the transition process according to state regulations to ensure alignment with Individualized Education Plans ( IEPs ) .

    + ensuring compliance with NYS laws , regulations , and district policies .

    Qualifications/Certifications :

    + Teacher Certification 7-12 - required

    + Work Based Learning Extension - preferred

    Key Responsibilities :

    Below are anticipated duties . The exact responsibilities may vary depending on specific student needs .

     

    + Transition Planning & Student Services

    + Facilitate collaboration with students , families , special education staff , counselors , and other relevant stakeholders to develop meaningful transition plans ( as part of IEPs ) , including long - term adult outcomes , employment goals , postsecondary education / training , independent living , and community participation .

    + Conduct person - centered transition assessments and vocational interest / preference assessments to help determine appropriate work - based learning placements .

    + Monitor progress toward transition goals and objectives , and prepare periodic reports .

    + Work - Based Learning ( WBL ) Coordination

    + Identify , develop , and maintain meaningful work - based learning sites ( internships , job shadowing, work experiences, etc.) that align with students' skills, interests, and IEP goals.

    + Match students to WBL placements , coordinate logistics ( transportation , schedules , mentorship ) , monitor student performance at worksites , and advocate / support students and employers .

    + Track and maintain documentation of student work - based learning hours , performance , evaluations , and completion .

    + Agency Referrals & Application Processes

    + Assist students and families with referral processes and application procedures for state and local agencies, especially ACCES-VR (New York State Office of Adult Career and Continuing Education Services - Vocational Rehabilitation) and similar services.

    + Monitor eligibility , application status , IPE ( Individualized Plan for Employment ) development , follow-up, and ensure compliance with agency requirements and timelines.

    + Facilitate interagency collaboration and liaison work between school , VR , adult agencies , community organizations , etc.

    + Skills Preparation & Student Support

    + Provide ( directly or coordinate ) instruction in employability skills : résumé writing , interview skills , job search strategies , workplace behaviors ( " soft skills " ) , self - advocacy , self-determination, financial literacy, etc.

    + Provide coaching / support to students while in WBL settings .

    + Assist families in understanding postsecondary options and navigating transitions from school to adult services .

    + Data , Monitoring , Compliance & Reporting

    + Monitor and track data related to student outcomes , WBL hours , placement success , follow - up after graduation , etc.

    + Maintain accurate and up - to - date records : student files ,, feedback / evaluations , etc.

    + Generate reports for internal review , district leadership , special education / CTE departments , and external agencies ( e.g. , NYSED , ACCES - VR) .

    + Partnership Development & Community Engagement

    + Build and maintain strong relationships with local employers , industries , community agencies , postsecondary education institutions , and workforce development boards to create and sustain WBL and transition opportunities .

    + Facilitate and organize events such as job fairs , worksite tours , employer panels , transition nights , etc.

    + Serve as a resource / trainer for school staff , families , community partners about transition services , WBL best practices , agency resources , legal / regulatory requirements , etc.
