    The role you'll contribute** **:

    The Patient Representative provides continuous and well-rounded coordinated support to surgical and non-surgical patients. Supports in educating the patient and/or family on pre-operative and post-operative care in addition to the treatments being provided in the practice. Serves as a liaison between the patient and practice support staff, also as a liaison between referring physicians and practice physicians. Screens and triages the patients' course of treatment incoming calls and communicates pertinent information to the physician and/or physician extenders. Displays a caring and professional attitude at all times as well as maintaining personal and professional growth.

    The value you'll bring to the team** **:

    Triages incoming calls from facilities, patients, physician offices, hospitals, pharmacies, and home health.

     

    Coordinates new patient appointments and disseminates information based on acuity level.

     

    Educates and screens all new patients in preparation for their appointments to ensure their plan of care is appropriate.

     

    Navigates patients through their entire plan of care.

     

    Other duties as assigned.

    Qualifications

    The expertise and experiences you'll need to succeed** **:

    High School Grad or Equiv - Required

    1 to 2 years of scheduling patients or administrative work in healthcare - Required

     

    Bilingual (Spanish) - Preferred

     

    Experience with PACS and Powershare- Preferred
