Position Summary:

We are seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to join our Virtual Palliative Care team. This role provides high-quality, patient-centered care to individuals with serious illness through telehealth platforms. The NP/PA will work collaboratively with an interdisciplinary team to support patients and families in managing symptoms, understanding their illness, and aligning care with their goals and values.

Key Responsibilities:

+ Conduct virtual palliative care consultations and follow-ups via secure telehealth platforms.

+ Perform comprehensive assessments including physical, emotional, psychosocial, and spiritual needs.

+ Develop and implement individualized care plans in collaboration with patients, families, and the care team.

+ Provide symptom management, advance care planning, and goals-of-care discussions.

+ Coordinate care with primary care providers, specialists, and other members of the healthcare team.

+ Document all patient interactions in the electronic medical record (EMR) in a timely and accurate manner.

+ Participate in interdisciplinary team meetings and contribute to continuous quality improvement.

+ Educate patients, families, and caregivers on disease progression, treatment options, and supportive resources.

Qualifications:

+ Current licensure as a Nurse Practitioner or Physician Assistant in NY

+ Minimum of 2 years of clinical experience in palliative care, hospice, geriatrics, or chronic disease management.

+ Experience with telehealth platforms and virtual care delivery preferred.

+ Strong communication, empathy, and interpersonal skills.

+ Ability to work independently and collaboratively in a remote environment.

Work Environment:

+ Fully remote

+ Requires reliable internet connection and private workspace.

+ Occasional travel for team meetings or training may be requested.

Salary- $133,900 - $156,200 Annually
