    Description

    JOB PURPOSE: The Data Coordinator collects, organizes, maintains, and reports all program data for the PPC program to track patient outcomes and meet contract requirements [0]. The Data Coordinator collaborates with the Program Manager to ensure data integrity, which supports quality improvement initiatives and the program's goals of enhancing health equity and improving patient outcomes throughout the HIV care continuum.

    MAJOR RESPONSIBILITIES (Essential functions):

    Result #1: Data Entry & Management:

    + Accurately and promptly enter all required patient data, including demographics, services provided, and core medical indicators, into eCW and the AIDS Institute Reporting System (AIRS).

    + Maintain and update patient records within AIRS, ensuring that all service encounters (intake, assessment, case conferences, referrals, etc.) are documented according to program standards and timelines.

    + Perform regular data audits and quality checks to ensure the completeness, consistency, and accuracy of all data.

    + Ensure data integrity, privacy, and security compliance (e.g., HIPAA standards).

    + Assisted in developing training materials, such as a manual on documentation, and provided on-the-job training sessions to new and existing users.

    Result #2: Data Reporting & Extraction:

    + Assist the Program Manager in preparing and submitting monthly program narratives by the 10th of each month.

    + Create routine and ad-hoc reports from eCW and patients' dashboards to monitor program performance against goals, including retention in care, viral suppression, and Social Determinants of Health (SDOH) metrics.

    Result #3: System Administration & Support:

    + Alongside the Program Manager and the Medical Care Management Specialist, serves as the program expert on AIRS, providing training and support to clinical and case management staff on data entry protocols and procedures.

    + Ensure compliance with NYS HIV confidentiality laws and HIPAA regulations in all data handling activities.

    + Perform, execute, carry out, or assist with data queries and backup procedures as outlined in program policies.

    Result #4: Quality Improvement & Analysis:

    • Support the program's Quality Improvement (QI) activities by providing accurate data for performance indicators.

    • Collaborate with the multidisciplinary team, including the Care Manager, Care Coordinator, RAP Specialist, medical providers, and peers, to ensure a seamless flow of information.

    • Perform other duties as assigned.

    Requirements

    EDUCATION AND EXPERIENCE REQUIRED:

    Option 1: Bachelor’s degree in public health, Information Technology, Social Science, or a related field, and a minimum of two (2) years of experience collecting, organizing, and maintaining data for projects.

     

    Option 2: Associate's degree and at least four (4) years of experience as described above.

     

    • Demonstrated proficiency with data collection and reporting software; experience with AIRS is a significant plus.

    • Bilingual in Spanish and English

    • Strong attention to detail and commitment to data accuracy.

    • Excellent organizational skills with the ability to manage multiple tasks and meet strict deadlines.

    • Understanding of and commitment to maintaining confidentiality and data security standards.

    • Basic analytical skills with the ability to summarize data for decision-making.

    • Preferred Qualifications:

    • Experience working in healthcare, public health, or non-profit settings, particularly with HIV-related services.

    • Familiarity with New York State Department of Health systems and reporting requirements.

    • Knowledge of health equity concepts, cultural competence, and the social determinants of health.

    • Experience with Electronic Medical Records (EMR) systems.

    SKILLS AND KNOWLEDGE REQUIRED:

    • Strong writing and computer skills

    • Possess and utilize effective verbal and written communication

    • Ability to be self-motivated, work independently, and work as a team member

    Salary Description

    $21.55 - $24.00
