    Community Services Provider

     

    TheCommunity Services Provideris responsible for providing direct care through implementation of Habilitation Plans. Specific duties and responsibilities include, but are not limited to:

     

    + Provide supervision and all direct care required to maintain safety and comfort of the person receiving services

    + Transport or accompany people receiving services on trips to community outings, appointments, or recreation activities for the purpose of community integration and exploration

    + Development and planning of inclusive community activities

    + Establish and maintain positive relationships with area businesses

    + Assist people receiving services with completing daily or routine domestic or personal care tasks

    + Foster and promote skills leading to independent living

    + Assist with the development of Habilitation Plans and following these plans for training and assistance

    + Provide training and assist people receiving services to develop, maintain and/or improve desired individual goals, living skills and personal needs as assigned or required

    + Provide supervision to people supported during program hours and during community outings as to ensure their health and safety

    + Complete required documentation in conformance with rules, regulations, policies and procedures, and billing practices

    SPECIAL REQUIREMENTS:

    + Possess good interpersonal, written, oral, and communication abilities and effective time management/organizational skills

    + Be computer literate with knowledge in agency operating systems and applicable software required for department needs.

    + Have regular access to internet

    + Demonstrate sound judgment in normal work and emergency situations

    + Must physically be able to perform required certifications and lift 50 pounds

    Requirements

    + High School Graduate/GED with some knowledge of or experience working with developmentally disabled preferred.

    + Possess a valid driver’s license acceptable to the agency’s insurance carrier
