Team 6 Covers: Bayport, Bellport, Blue Point, East Patchogue, Holtsville, Medford, Patchogue, South Fork, Brookhaven, Bellport, Center Moriches, East Moriches, East Quogue, Eastport, East Yaphank, Hampton Bays, Manorville, Mastic, Mastic Beach, Moriches, Quogue, Remsenburg, Sag Harbor, Shinnecock Hills, Shirley, Southampton, Speonk, Westhampton, & Westhampton Beach

+ 5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekends

Job Details

Our Home Care PTs:

+ Review patient charts and medical history

+ Develop a personalized treatment plan

+ Teach therapeutic exercises and stretches to improve patient’s conditions

+ Use hands-on therapy to promote healing

+ Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly

+ Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload.

+ Provide patient and family education for health promotion as directed by physician’s plan of treatment.

+ Provide care to the most upmost standard

Minimum Qualifications:

+ New York State PT license

+ New Grads welcomed

+ Valid New York State Driver’s License and reliable transportation

Salary Range

USD $94,000.00 - USD $120,000.00 /Yr.

The pay for this position ranges from $94,000 -$120,000 Annually.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
