Texas Health Huguley- operates as a joint venture between Texas Health Resources and AdventHealth.
**Schedule:** Full-Time
**Shift:** Monday - Friday (8:00am - 5:00pm)
***Responsible for traveling to all other Adventhealth hospitals in Texas periodically - Texas Health Hospital Mansfield, Adventhealth Central Texas, Adventhealth Rollins Brook)

The role you'll contribute:
The Care Management Market Director has full accountability and oversight for the overall operations of Care Management within their assigned market. The Care Management Market Director reports directly to the Division CNE and has a dotted matrix reporting relationship to the Corporate Executive of Care Management. The Care Management Market Director, in partnership with the Divisional/Regional CNE, works directly with Facility Chief Nursing Officers and Facility Care Management Directors to guide and monitor them in implementing the Corporate Care Management strategies.
The Care Management Market Director is responsible for the direction, supervision and evaluation of the operations of Care Management for their assigned market. The Care Management Market Director provides strategic guidance to achieve goals in an evolving healthcare environment including, but not limited to, population health and provider-risk models. The Care Management Market Director promotes quality improvement, and coordinates activities with other departments to provide and maintain optimal standards of patient care delivery. The Care Management Market Director is responsible for ensuring staff adhere to the Division/Region/Market and Corporate Compliance Programs, Hospital facilities' operational policies and procedures; applicable post-acute care policies and procedures, rules and regulations of all local, state, and federal guidelines, and standards of accrediting bodies.
The Care Management Market Director ensures principal duties and job responsibilities are achieved through the supervision and direction of the Care Management Facility Directors. The Care Management Market Director establishes excellent working relationships with the Division/Region CMO and CFO and facilitates bidirectional communication of progress, barriers and updates. The Care Management Market Director will review monthly metric reports and operational improvement plans from each of the facilities and provide a summary to divisional/regional leadership.

+ Leads the execution of the strategic vision for care management across the organization, enhancing the consumer experience and deploying Care Management resources effectively.
+ Develops strategies to reduce length of stay, readmissions, and clinical denials while simplifying daily workflows and supporting improved governance and accountability structures.
The expertise and experience you'll need to succeed:
Minimum qualifications** **:
+ Master's of Nursing OR
+ Master's in Social Work OR
+ Master's in related field
+ 10 Years Work Experience
+ 5 Years Leadership Experience
Preferred qualifications:
+ Registered Nurse (RN) if Registered Nurse
+ Certification in Care Management Standards or ACM or CCM
