Position Purpose:

Join Trinity Health At Home as a Home Care Occupational Therapist ( OT ) and make a meaningful impact by delivering compassionate, high-quality therapy services to clients in their homes. You’ll help individuals regain independence and improve their quality of life through personalized care plans and therapeutic interventions.

Coverage Regions:

Romulus, Bellville, Van Buren, Whittaker, Willis, and New Boston

Dearborn, Dearborn Heights, Garden City, Livonia, Taylor, Wayne, Westland, Redford

Oakland County: Waterford & Pontiac

Livingston County: Brighton, Chelsea, Dexter, Howell

What You Will Do:

+ Assess clients’ functional status and develop individualized treatment plans in collaboration with physicians.

+ Deliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.

+ Educate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.

+ Supervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).

+ Coordinate care with interdisciplinary teams and participate in case conferences.

+ Document all care activities accurately using point-of-care systems.

Minimum Qualifications:

+ Graduate of an accredited Occupational Therapy program.

+ NBCOT certification and state licensure/registration.

+ Minimum one year of experience or completion of a 6–8 week preceptorship.

+ Valid driver’s license and reliable transportation.

+ Home health experience preferred.

+ Commitment to ethical standards and continuous learning.
