Position Title: Case Manager
Program: Older Adult Services – Austin Street Site
Status: Full-Time
Schedule: Monday – Friday, 9:00 AM – 5:00 PM (35 hours per week)
Compensation: $65,000 per year

Queens Community House (QCH) is a multi-site nonprofit organization serving the diverse population of Queens through a broad network of programs for children and youth, adults and families, and older adults. We are seeking a Case Manager with a strong commitment to social services and a passion for working with older adults. Case Management services play a vital role in assisting older individuals with functional impairments by helping them access the appropriate services, benefits, and entitlements necessary to age safely at home while maintaining their quality of life.
The Case Manager will manage an average caseload of approximately 70 older adults residing in Community Districts 4 and 6, which include Elmhurst, Corona, Forest Hills, and Rego Park. The Case Manager will provide professional, timely, and culturally sensitive support while actively engaging clients and caregivers in a collaborative, problem-solving process to meet their unique needs.
Primary Responsibilities
+ Screen potential clients through the intake process; assess presenting issues such as personal and family adjustments, finances, and physical or mental impairments.
+ Provide clients with accurate and appropriate information on services, benefits, and entitlements.
+ Conduct in-home assessments and re-assessments to determine eligibility and continuation of services.
+ Develop and implement individualized, culturally sensitive care plans that efficiently utilize community resources.
+ Identify and engage clients’ informal supports to encourage ongoing participation and assistance.
+ Serve as a liaison with external service providers; arrange for service delivery, advocate, and intervene on behalf of clients as needed.
+ Maintain complete and accurate case records, ensuring timely documentation.
+ Collaborate with colleagues and develop a working knowledge of all QCH services, making cross-department referrals as appropriate.
+ Assist clients with the utilization of Supplemental Services Funds, including identifying needs and coordinating purchases and delivery.
+ Support outreach efforts as needed to promote program awareness.
+ Participate in NYC Department for the Aging and in-house or external training to fulfill required annual training hours.
+ Attend staff meetings, participate in QCH’s annual Staff Development Day, and engage in regular supervision.
+ Complete annual performance evaluations and contribute to a positive, collaborative team environment.
+ Perform other duties as assigned.
Qualifications
+ Bachelor’s degree required; Master’s degree in Social Work (MSW) or related field (e.g., Social Services, Public Administration, Nursing, or Public Health) preferred.
+ Minimum of two (2) years of experience working with older adult populations.
+ Bilingual – Russian required.
+ Demonstrated cultural competency and sensitivity to the diverse needs of older adults in the community.
+ Knowledge of the communication needs of individuals with visual and/or hearing impairments.
+ Strong organizational skills with the ability to multi-task and manage priorities effectively.
+ Proficiency in computer applications; experience with the Senior Tracking, Analysis, and Reporting System (STARS) preferred.
+ High level of professionalism and discretion when handling sensitive or confidential information.
+ Strong interpersonal and communication skills with a customer-service orientation.
+ Detail-oriented, with the ability to analyze data, solve problems, and follow through on assignments.
