Long-Term Acute Care(LTAC) Registered Nurse (RN)

The LTAC RN is a dedicated and skilled nursing professionalresponsible for providing comprehensive care to medically complex patientsrequiring extended hospitalization and specialized treatment. This role demandsstrong clinical judgment, adaptability, and the ability to manage multiplepriorities in a dynamic environment. The RN collaborates with physicians,interdisciplinary teams, and support staff to ensure safe, effective, andpatient-centered care focused on long-term recovery.

Essential Duties and Responsibilities

• Apply the nursing process to assess, plan,implement, and evaluate care for patients with chronic, critical, andpost-acute conditions.

• Conduct thorough patient assessments, includingvital signs, physical exams, health histories, and monitoring of complexclinical parameters.

• Manage advanced therapies such as ventilatorsupport, tracheostomy care, and complex wound care.

• Administer medications, treatments, and IVtherapies as prescribed.

• Monitor and document patient responses tointerventions and changes in condition.

• Collaborate with healthcare providers to developand adjust individualized care plans focused on long-term goals.

• Prepare patients for diagnostic tests,procedures, and transitions of care.

• Provide education to patients and families ondisease processes, medications, discharge planning, and self-care strategies.

• Maintain accurate and timely documentation inthe electronic health record (EHR).

• Adhere to infection control, safety, and qualitystandards.

• Respond promptly to emergencies such as rapiddeterioration, respiratory distress, or code situations.

• Advocate for patients and uphold patient rightsand confidentiality.

Minimum Requirements

• Current Registered Nurse License in the state ofpractice.

• Minimum one year of professional nursingexperience; LTAC, ICU, or Med-Surg experience preferred.

• Completion of Amergis' Competency Program (ifapplicable).

• Current BLS certification (ACLS preferred).

• Proficiency in ventilator management,tracheostomy care, and complex wound care.

• TB screening and health clearance (as requiredby contract or regulation).

• Compliance with all federal, state, and localregulations.

• Successful completion of new hire orientationand LTAC-specific training.

• Understanding of patient confidentiality andHIPAA regulations.

• Strong communication skills and proficiency inEnglish.

• Basic computer literacy.

• Must be at least 18 years of age.
