Position summary: The outreach program coordinator will PROVIDE administrative oversight, strategy, design and task completion for the LI Care Coordination division and the children and family division
ESSENTIAL ACCOUNTABILITIES:
Responsible for establishing and implementing strategies and guidelines for outreach and marketing of the agency LI care coordination programs and the children and family array of services.
The Outreach Program Coordinator directly supervises outreach and marketing, and the Care Management staff (full time/part time) who are performing outreach activities.
Responsibilities include, but are not limited to:
+ + Must demonstrate knowledge of the philosophy, approach, expected outcome of Care Coordination and services for children and families.
+ Having a working understanding of the regulations of the Health Homes Serving both Adults and Children and of the array of services available to children and families.
+ Must have a working knowledge of the system of care for both adults and children to enhance matching of service with consumer needs
+ Responsible to create and implement a marketing plan for all service divisions/programs assigned.
+ Responsible for ensuring compliance with all DOH, OMH and Health Home Regulations regarding enrollment for services. Remain current with any changes instituted.
+ Demonstrate and implement skills of organization, prioritization, creative problem solving, brainstorming to maximize outreach and engagement.
+ Participate in the recruitment and hiring process of staff.
+ Provide ongoing supervision of staff regarding outreach and marketing ensuing follow up to all contacts as appropriate
+ Meet metrics as agreed upon for each component of outreach/marketing
+ Monitor paperwork of all outreach staff to ensure compliance with Health Home, MCO and agency requirements
+ Prepare montly reports for the Senior Business Director and Division Directors as assigned
+ Participate in meetings (supervisory as scheduled and to address issues as determined by acuity) as scheduled with Senior Business Director and Division Directors
+ Manage calendars to ensure all events, appointements are covered and supervisees are maximizing time
+ Perform data analysis of activities to ensure efficiency and effectiveness
+ Address questions of prospective clients and of staff; leadership and direct care
+ Organize and attend community outreach events to represent the adult and children's divisions
+ Attend meetins as appropriate in Divisions, conduct outreach staff meetings routinely at scheduled intervals determined by need and agreed upon by Senior Business Director and Division Directors
+ Participate in trainings as they enhance job effectiveness
OTHER RESPONSIBILITIES:
+ Represent the agency at meetings and trainings not otherwise specified.
+ Ability to work flexible schedule as work schedule and locations are subject to change.
+ This does not limit the assignment of duties or exclude the performance of other duties not mentioned, as assigned by supervisor.

Qualifications
Qualifications:
A Bachelor’s degree, with two years of experience in the Social Service field. Computer literacy is necessary. Familiarity with business planning, strategies for marketing is preferred. New York State driver’s license and access to a vehicle required .
