MDS Coordinator (RN) – Long-Term Care | Dubuque Specialty Care

Are you an experienced Registered Nurse (RN) with a passion for clinical excellence and resident care planning? Dubuque Specialty Care, part of Care Initiatives , is hiring a skilled MDS Coordinator (RN) to oversee the assessment process and ensure accurate documentation for our residents. What You’ll Do as an MDS Coordinator:

+ Lead and manage the resident assessment process to ensure accuracy and compliance

+ Collaborate with interdisciplinary teams to develop individualized care plans that improve outcomes

+ Monitor and evaluate resident progress, adjusting plans as needed

+ Serve as a clinical resource for staff and a liaison for residents and families

+ Ensure compliance with federal, state, and facility regulations

Qualifications:

+ Active RN license in Iowa (or ability to obtain/transfer)

+ Previous experience in long-term care highly preferred

+ Current CPR certification
