Position Summary

To provide in-home therapy, supportive counseling, and service linkages for individuals over the age of 60 who are experiencing emotional distress.

Primary Position Responsibilities

Provides direct services in a clinically appropriate and sensitive manner.

Demonstrates knowledge of entitlement programs.

Completes all Hunterdon Older Adult Program (HOAP) documentation.

Interacts and collaborates with healthcare departments, community providers, and families.

Qualifications

Minimum Education:

Required:

Master’s Degree in Social Work, Counseling or Psychology

Minimum Years of Experience (Amount, Type and Variation):

Required:

2 years’ experience working with the elderly population + knowledge of caregiver issues

Preferred:

3+ years providing counseling and service linkage to elderly population including

License, Registry or Certification:

Required:

Valid Driver’s license

Knowledge, Skills and/or Abilities:

Required:

Must be able to relate well to senior adults, service providers, medical professionals both in hospital and in community, and family members. Must have ability to facilitate groups. Knowledge of the aging process and caregiver issues. Familiar with Hunterdon County and services provided to seniors, Must be ability to work with minimal supervision and conduct in-home visits.
