    PURPOSE

    The I-CARE Case Manager reports directly to the Associate Dean of Students (ADOS) and serves on the I-CARE Behavioral Intervention Team. The position works to foster student development and to offer a supportive, personalized response when difficulties arise. They are responsible for case management and work closely with the ADOS to address and coordinate the needs of students who have health, psychological, economic, family relationships, and/or social adjustment-related problems through a variety of interventions, referrals and follow-up services. They provide consultation to campus community and are responsible for developing partnerships across campus and with local agencies in the community to provide and connect students with resources and services.

    DESCRIPTION

    Functional Area 1: Case Management

    Percent Effort: 45%

    + Takes Care Referrals, investigates concerns, evaluates, and presents cases to I-CARE Team.

    + Serves as a point of contact and provides consultation for campus community members who are seeking guidance about services for students.

    + Manages referrals to on-campus and community-based resources. Works closely with campus constituents in coordinating referrals and services.

    + Assists students in understanding policies and procedures relevant to their situation and works to identify solutions for the student.

    + Coordinates follow-up and tracking of students in distress to monitor progress. Completes documentation and maintains record keeping of cases in Maxient.

     

    Functional Area 2: Administrative

    Percent Effort: 35%

    + Assists the ADOS with the updates and maintenance of the Care & Assistance Manual, the website, community resource provider list and marketing materials.

    + Serves as a back-up to the I-CARE Team in the absence of the Chair.

    + Serves on departmental and campus committees and projects.

    + Collaborates with Graduate Assistants within Dean of Students Office.

    + Performs other duties as assigned.

     

    Functional Area 3: Outreach

    Percent Effort: 20%

    + Facilitates training sessions and presentations for the campus community.

    + Collaborates with case managers on campus to resolve student issues and expand campus resources.

    + Establishes working relationships with community mental health providers and other support agencies.

    QUALIFICATIONS

    + Bachelor’s degree in applicable field.

    + Five (5) years of related experience.

    + Additional education may be used as a substitute for the minimum experience requirement:

    + Master’s degree and three (3) years related experience

    + Doctoral degree and one (1) year related experience

    + Experience in crisis management and response.

    + Experience in facilitating training and/or presentations.

    + Intermediate computer skills in Microsoft Word, Excel, PowerPoint, and Outlook.

    + Ability to develop and maintain case management files, referral resources and community services database.

    + Ability to maintain confidentiality based on relevant laws, policies and/or procedures.

     

    SALARY $4,166 Monthly ($50,000 Annually Approximately)
