Position Purpose
The Program Coordinator works in collaboration with the Trinity Health Director of Clinical and Social Care Integration and MGPS Director to ensure Trinity Health is fulfilling its commitment to promote optimal health for poor and vulnerable populations and communities we serve by 1) scaling chronic disease prevention and self-management programs system-wide. This position acts as a strategic thought partner, operationalizing the clinical-social care programs and facilitating relationship-building between Community Health & Well-Being (CHWB) and ambulatory care providers. This position leverages resources and partnerships, focusing on providing optimal, accessible care to poor and vulnerable patients, particularly focusing on those populations served in Trinity Health Safety Net Health Centers (SNHC).
This position develops effective internal and external relationships and partnerships with various key stakeholders, management, executive leadership and several multi-functional and geographically dispersed work teams by providing direction, assistance, coordination and collaborating to synchronize CHWB and ambulatory care provider efforts.
What you will do
+ Establish working relationship with MGPS/CHWB team to build on existing organizational efficiency assessments and create custom time bound workplans for aligning with CIN best practices
+ Consult with Medical Group leaders and regional ambulatory care directors on strategy, measuring success, operations and implementation requirements
+ Provide coaching and develop technical assistance tools and resources for chronic disease prevention and intervention
+ Seek and manage grant opportunities that further the Chronic Disease prevention strategy
+ Analyze healthcare landscape to identify potential opportunities for advancement of SNHC and lead engagement of national and community collaborations, innovative community interventions, academic and industry partners, medical respite and mobile health units, and Social Influencer of Health policy
+ Manage initiatives and programming within the ADAPT and other disease prevention/health promotion programs
Minimum Qualifications
+ Bachelor's degree in Health Care Administration, Public Health, Community Health or a related field with one (1) to three (3) years of experience community health, public health, social services, or an equivalent combination of education and experience
+ Must possess a seasoned knowledge of health equity, Social Influencers of Health, and health care delivery
+ Advanced experience with project management, strategy development and organizational change management in a healthcare environment
