    Empowering People's Independence (EPI), provides support for people with developmental disabilities, epilepsy, and brain injury through progressive, tailored care to improve their health, wellbeing, independence, and social inclusion.

    Job Purpose:

    The primary purpose of this position is to manage the administrative and logistical aspects of the Coordinated Assessment System (CAS) to include ensuring the needs of individual’s supported are clearly identified, training and serving as point person/liaison for all involved parties during the Assessment process.

    Essential Functions:

    Administrative:

    + Collaborates with Care Plan teams of Residential and Community Day programs, to ensure individual’s needs are reflected accurately in electronic records and are consistent with the CAS tool.

    + Attends care plan meetings to ensure understanding of individual’s needs.

    + Periodically audits individual’s records and resolves discrepancies of records to ensure accurate reflection of individual’s needs.

    + Prepares families and care team for CAS interviews to include explaining purpose of process, ensuring clear understanding of individual’s needs, familiarity with records and their role in the process.

    + Ensure a smooth and efficient workflow of the CAS process, to include coordinating assessment interviews with individuals, families, Care Coordination Organizations (CCO’s) and Office for People with Developmental Disabilities (OPWDD).

    + Ensure the interview process runs efficiently and meets all program objectives and productivity targets.

    + Identify and resolve any issues that arise, to include coordinating last minute scheduling changes, clarifying information, etc.

    + Coordinate any appeals of the CAS, if needed, with Care Manager and Team

    + Provide ongoing training to care teams and families on CAS process and their role.

    + Report any issues to Director of Residential Program and/or designee.

    Record Keeping & Reporting:

    + Ensure individuals records accurately always reflects individuals’ needs.

    + Track new services and manage assessments of all existing individuals to ensure needs of the individuals are clearly identified.

    + Generates reports based on observations of individual and/or family/care team meetings.

    + Provides feedback and analysis on CAS process.

    + Develops Procedures and Training Manuals for care team & family.

     

    Other

     

    Collaborates with Finance on budget related matters.

     

    Performs other duties as assigned by Director of Residential Program and/or designee within scope of practice.

    Non-Essential Functions:

    + Presents to Leadership Team as requested.

    + Participates on various EPI committees as able.

    Compliance :

    + Must maintain confidentiality and exercise discretion concerning all business-related matters, including but not limited to board/vendors/donors’ names, investigations, finance, operations, and employee relations matters.

    + Must be willing to complete specific company required trainings as mandated by our governing agency, Office for People with Developmental Disabilities (OPWDD), at time of hire and annually thereafter.

    + Must adhere to all departmental /company policies & procedures at all times.

    Education and Experience:

    Minimum Associate’s degree required with 3-5 years’ experience working in intellectual and/or developmental disabilities (I/DD) in residential and/or community programs. Bachelor’s degree preferred with minimum 3 years’ experience with progressively demanding and complex job assignments within a residential and/or community program. Familiarity with OPWDD system, CCO’s and CAS is required.

    Computer Skills:

    Competent with the MS Office Suite applications to include Word, Excel, Outlook & Teams).

     

    Ability to navigate electronic health record applications (e.g., Medisked, eVero, Choices, etc.).

     

    Demonstrated ability to learn new software programs quickly.

    Qualifications:

    + Must possess and maintain a valid NYS Driver’s License-a vehicle is required for business travel up to five (5) days per week throughout Broome, Monroe & Wayne counties, with most of the travel in Monroe County.

    + Demonstrated ability to use critical thinking skills.

    + Solid verbal and written communication skills; ability to communicate with all persons supported and a diverse employee population on various levels within the organization.

    + Detailed oriented.

    + Demonstrated ability to use poise, tact, maturity, and diplomacy, during difficult and/or stressful situations to effectively collaborate and resolve issues.

    + Proven ability to identify and logically resolve complex problems.

    + Proven ability to influence others for positive outcomes.

    + Must be able to thrive in a fast-paced environment, with demonstrated ability to manage multiple tasks, and customer requests, while meeting deadlines.

    + Demonstrated ability to work independently and collaborate effectively with diverse teams.

    Physical Demands:

    + The physical demands described below are representative of those that are required to successfully perform the essential functions of this job. Individuals with disabilities, which impact their ability to perform the essential functions, will need to review reasonable accommodations with Human Resources.

    + Must be able to talk, hear, stand, walk, and/or sit for extended periods of time; may have to twist, bend, use hand to finger, stretch with hands and arms.

    + Specific vision abilities required by this job include close vision and ability to adjust focus.

    + May be exposed to strong odors from time to time.

    + Must be able to lift and carry up to (50) lbs.

    + While performing the duties of this position, exposure to changing weather conditions will occur.

    $63,500-$69,500-$75,500
