Job Summary and Qualifications
The Stroke Services Coordinator ensures quality, timeliness and effectiveness of stroke care delivery for Osceola Regional Medical Center, and is responsible for: program assessment, planning, organizing, implementation, budgeting, performance improvement, education, stroke program hospital services utilization, community outreach, pre-hospital education, and marketing/communications. The Coordinator also works in a multidisciplinary role to promote quality in clinical practice for a specific patient population with the goal of providing superior quality, competitive value, and outstanding service to patients, families and the community.
+ Coordinates and provides direction to the BRRH Stroke Center and oversees the systems and programs that are involved in stroke care in collaboration with the Stroke Center Medical Director.
+ Works collaboratively with the medical staff and hospital departments to assure there are effective Stroke Care services in place.
+ Works collaboratively with hospital neurologists and emergency room physicians to provide well-coordinated services to the patient.
+ Reviews and maintains policies and procedures specific to the Stroke Center.
+ Coordinates and provides direction to the facility's stroke care team including communication and coordination with the medical staff, quality, nursing and others as assigned by providing accurate and timely information and written communication.
+ Acts as co-chair or facilitator of the stroke committee and stroke care improvement teams
+ Documentation and dissemination of minutes
+ Preparation of agendas
+ Completes follow up to open items
+ Accurate reporting to other committees as required
+ Maintains the Stroke Center PI Plan in conjunction with the Quality Improvement Department
+ Provides accurate and timely clinical data to appropriate committees or groups and identifies opportunities for improvement in conjunction with the Quality Improvement Department.
+ Collect data or oversee processes for data collection
+ Aggregate and analyze data
+ Provide Benchmarks
+ Verify Statistical measures
+ Understands databases and data collection methodologies specific to the stroke program
+ Accountable for development of performance improvement and action plans In conjunction with the Stroke Center Medical Director coordinates and provides support to the clinical peer review process for stroke center patients:
+ Accurate and timely chart review and documentation
+ Follow up with specific physicians
+ Develop focus studies as needed
+ Acts as a resource with regulatory agencies and makes recommendations for compliance and maintains mandatory documentation as required.
+ Joint Commission Disease Specific Standards
+ AHCA Primary Stroke Standards
+ Stroke Alert indicator compliance Maintains responsibility for knowledge and skill of clinical requirements in nursing specialty.
+ Responsible for initial and ongoing education programs and development of content for Stroke Center caregivers in compliance with compliance with mandatory standards.
+ Oversees content of education programs and materials
+ Ensures timeliness of education schedules and program content
+ Coordinates with the Center for Learning for Program scheduling and instruction
+ Oversees EMS and community education programs and mandatory requirements
+ Functions as a role model in the use of nursing process focusing on best practice initiatives in stroke and neurological nursing care. Serves as a resource to the BRRH Neuroscience and Primary /Stroke programs.
+ Develops and implements a comprehensive plan of care for the stroke patient through multidisciplinary collaboration, as well as serving as an active participant in the hospital's research process
What qualifications you will need:
+ Current Florida RN license, or Florida eligible
+ BLS provider required
+ ACLS provider within six months
+ National Certification in area of specialty preferred
+ Two to five years of stroke patient care nursing experience as an RN with an emphasis in critical care
+ Previous stroke nursing education experience preferred
+ Excellent knowledge and understanding of stroke patient care in a complex acute care setting
+ Demonstrated success in program development preferred in a leadership role
+ Knowledge with stroke data bases and Joint Commission standards for primary stroke certification
+ Stroke committee participation
