POSITION SUMMARY:
Assists in the coordination of activities that promote individualization, independence, inclusion and productivity for persons receiving services. Acts as a resource to the Program Coordinator in the planning and development of activities in and out of the residences. Coordinates transportation arrangements for planned activities.
POSITION DUTIES:
+ Explores recreational opportunities in assigned borough and establishes and maintains a database that includes information regarding types of activities offered, description of activities, accessibility, costs, etc. to the persons receiving services.
+ Assists the Program Coordinator in the identification and utilization of natural supports and community resources.
+ Responsible for establishing and facilitating Special Interest Clubs.
+ Coordinates and/or provides transportation for planned activities and for all assigned residences.
+ Plans and coordinates special events for assigned residences.
+ Acts as a resource for Residential Coordinators and Program Coordinator in the planning of activities in and out of the residences including vacations.
+ Assists the Residential Coordinator in ordering activity supplies for all assigned residences and suggests a list of activities for each Site Specific and the needs of the people at the site.
+ Provides in-service training to direct care and management personnel in the area of community inclusion and the completion of the Community Inclusion form.
+ Coordinate and Facilitates Borough Wide Self-Advocacy meetings. (For more than one Borough may be requested.)
+ Reviews and signs community inclusion logs for all assigned residences.
+ Assists the Administrator with monitoring the recreation budget for all assigned residences.
+ Attends management and cluster meetings as needed.
+ Performs all other duties as assigned.
REQUIREMENTS:
+ Minimum Bachelor's Degree in a related field required.
+ Demonstrates ability to develop and implement activities that promote individualization, independence, inclusion, and productivity.
+ Minimum of two (2) years experience working with persons with disabilities required.
+ Must be able to work flexible hours including evenings and weekends as needed.
+ Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency’s corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
