    **What you’ll do:** You’ll provide care required by the resident identified with one or more chronic conditions utilizing established policies, procedures and treatment regimens under the direction of a licensed physician or mid-level provider in compliance with MTC Medical and TCCO directives.

    Essential Functions:

    1. Provide an environment conducive to safety for residents and employees. Assess risks to the residents' safety and implement the appropriate precautions.

    2. Maintain current master list of residents with on-going chronic medical needs.

    3. Maintain and update contraindications list.

    4. Ensure that residents are scheduled to appropriate clinician (physician or mid-level provider) and monitor medication compliance.

    5. Assist physicians or mid-level providers as required.

    6. Serve as unit resource and provide education as required to residents and staff.

    7. Prepare and maintain unit statistical reports for submission to HSA - statistical clerk.

    8. Maintain absolute security and confidentiality of all medical records; observe applicable HIPAA rules.

    9. Review medical files to determine all provided services are documented.

    10. Closely monitor all potential catastrophic illnesses.

    11. Provide health education on individual or group basis, as required.

    12. Direct and conduct monthly chronic clinic and regular meetings.

    13. Recommend methods of improving operational efficiency and cost effectiveness of health-related services.

    14. Attend scheduled staff meetings; promote communication and proper flow of relevant information in medical unit.

    15. Prepare and disseminate safety and accident reports as required by policy; take appropriate action in cases of serious and unusual incidents and emergencies.

    16. Perform rescue functions at accidents, emergencies and disasters to include administering basic emergency medical aid, physically removing people away from dangerous situations, and securing and evacuating people from confined areas.

    17. Accurately and promptly chart medical care.

    18. Regular and predictable attendance required.

    19. It is expected that the incumbent will work overtime when directed to do so and cover shifts when directed to do so.

    20. Promote the development of positive social skills through modeling appropriate behaviors and intervening when inappropriate behaviors are observed.

    21. Maintain accountability of residents and property; adhere to safety practices.

     

    It is expected that the incumbent shall perform other duties as assigned within his/her capabilities as determined by management.

    Education and Experience Requirements:

    Degree in nursing and valid registered nursing license for the state of Texas or compact state required. Must possess current CPR certification. Must work within the Texas state board of nursing scope of practice.

     

    One (1) year of experience, to include EKG, phlebotomy, injections, IV, knowledge, training, and/or continuing education courses in chronic illnesses preferred. Valid driver's license with an acceptable driving record required, unless waived by management.
