The Ambulatory Care Clinical Pharmacist provides evidence-based pharmaceutical care to patients in the ambulatory clinical setting. The delivery of care by the Ambulatory Care Clinical Pharmacist includes, but is not limited to: designing, implementing, assessing, monitoring, and documenting therapeutic plans utilizing best evidence and most effective and most economical medication treatments; helping to achieve positive patient outcomes through interactions with patients, providers, and interdisciplinary teams in the ambulatory clinical setting. When appropriate, pharmacists who provide ambulatory care services may perform patient assessments, participate in collaborative practice agreements to provide medication management, provide collaborative drug therapy management, order, interpret, monitor medication therapy-related tests, coordinate care and other health services for wellness and prevention of disease, provide education to patients and caregivers, and document care plans in the medical record.

_What You’ll Do:_
+ **_Deliver expert medication therapy management_** _for patients with complex neurologic malignancies in an ambulatory setting._
+ **_Collaborate closely_** _with neuro-oncology physicians, infusion center teams, and specialty pharmacy to optimize treatment outcomes._
+ **_Support advanced therapies_** _including oral oncolytics, immunotherapies, and investigational agents._
+ **_Drive continuity of care_** _across inpatient, outpatient, and infusion services—ensuring safe transitions and improved patient experience._
+ **_Enhance UVA prescription capture_** _through proactive engagement with UVA specialty and retail pharmacy services._
+ **_Educate and mentor_** _pharmacy learners and contribute to continuous professional development across the care team._

Minimum Qualifications
+ Education: Bachelor’s in Pharmacy or Pharm. D Degree required.
+ Experience: PGY-1 residency required or equivalent hospital experience. (3.years). PGY-2 residency is preferred for specialty areas.
+ Licensure/Certifications: Commonwealth of Virginia State Board of Pharmacy. License required. American Heart Association (AHA) Health Care Provider. BLS certification required.
**Physical Demands –** Job requires sitting for prolonged periods; Repetitive motion: typing. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull < 20lbs. May be exposed to fumes, chemicals, blood/body fluids and infectious disease.
**Compensation –** _Include for all roles that do not follow the standard comp grid_
+ The pay rate for this role starts at $64.37 per hour. New hire compensation will be determined by education and experience.
Onboarding Requirements
Background checks and pre-employment health screenings** will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a **drug screen.
Position Compensation Range: $55.67 - $89.07 Hourly
