Description
Position Summary:
Person in this position will be responsible for the case management and/or Navigation of clients enrolled in the Bridges to Success Adult Reentry program. The Case Manager will utilize the Housing First approach to the delivery of its Reentry services to formerly incarcerated individuals who are experiencing or are at imminent risk of homelessness in an effort to break the cycle of homelessness and incarceration. The services provided will assist in reintroducing formerly incarcerated individuals, including serious and violent offenders, back to their respective communities, family systems and other positive supports. This will include arrangement, coordination, monitoring, and delivery of services related to meeting the housing needs of the participant and helping them obtain housing stability. Essential Job Functions include but not limited to those listed below.
Essential Job Functions:
+ Provide case management services to a caseload of participants, up to 25 at a time.
• Navigate clients through Coordinated Entry System and see clients through to appropriate housing intervention
• Provide Rapid Rehousing supportive services to eligible participants and actively promote housing sustainability and self sufficiency
• Maintain solid communication with Bridges to Success Housing Locator to support housing sustainability for participants
• Serve as an active part of the Bridges to Success team and fulfill all required responsibilities as needed.
• Intake and assessment of incoming participants to determine eligibility and appropriate services.
• Manage of a caseload participant including maintenance of client files, development of services plans, and tracking pertinent data.
• Make referrals and appointments with appropriate agency, services including but not limited to health care providers, employment offices, substance and mental health treatment programs, public assistance, and other mainstream services.
• Client advocacy
• Assist clients with, completion of supportive and subsidized housing paperwork, and monitoring progress to completion
• Maintain client files according to company policy and contract requirements.
• Heavy documentation; WestCare’ s Clinical Data System
• Provide daily, weekly, and monthly reports as requested
• Assist and monitor data collection and statistical reporting and complete Quality Improvement reports as requested.
• Develop and maintain relationships with referring and supporting agencies and represent the agency at various community meetings as requested.
• Conduct community outreach and program presentations, as needed.
• Provide follow up after housing to insure client stability and successful outcomes.
• Must embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation.
• Other relevant duties as assigned.
Qualifications
Essential Qualifications:
+ Minimum two (2) years’ experience in social services, case management, or related human services field.
+ Strong knowledge of housing programs, supportive services, and community resources.
+ Excellent communication, public speaking, and interpersonal skills.
+ Strong computer skills, including Microsoft Office (Word, Excel, PowerPoint).
+ Good documentation and organizational skills.
+ Ability to work independently, manage multiple priorities, and remain calm in challenging situations.
+ Demonstrated ability to work effectively with diverse and underserved populations.
Certifications/Licenses:
• Valid driver’s license and ability to transport clients.
• CPR Certification, First Aid Certification, and annual Tuberculosis test.
Education:
+ Graduation with a two-year degree in Psychology, Social Work, Community and Human Services, Rehabilitation Counseling or a related field is required; and
+ High School diploma or equivalent is required.

+ Occasional lifting of more than 10 pounds;
