Essential Functions

1. Review, interpret, and advise on physician orders, providing formulary guidance and ensuring safe, effective medication use in alignment with hospice specified parameters
2. Provide expert pain and symptom management consultation, tailoring recommendations to individual patient needs and supporting patient comfort and dignity during end-of-life care.
3. Collaborate directly with nurses, hospice staff, and multi-disciplinary care teams to develop effective, cost-conscious medication care plans and coordinate timely responses in a fast-paced call center environment.
4. Educate healthcare professionals and caregivers on the appropriate drug therapy, medication administration, and strategies to prevent adverse drug events.
5. Document clinical interventions, encounters, and patient outcomes in electronic health systems, ensuring accuracy, clarity, and regulatory compliance
6. Conduct drug utilization reviews and maintain complete, compliant medication profiles, supporting medication safety and adherence to HIPAA and regulatory standards.
7. Demonstrate strong communication, empathy, and professionalism in all interactions within remote or hybrid ork environments.
8. Meet or exceed productivity expectations, monitor required queues (Infinity, phone, staffing), and perform phone audits as necessary.
9. Support team operations through collaboration, shared learning, and assistance during peak volumes, including providing status updates and addressing urgent matters with leadership.
10. Participate in policy and procedure development, ensure compliance, and contribute to coverage needs, including weekend rotations as assigned.

Knowledge, Skills & Abilities
Education, Licensure or Certification:
• PharmD or B.S. in Pharmacy from an ACPE-accredited school of pharmacy
• Active pharmacy license in home state (Pennsylvania or Tennessee)
• Pharmacy licenses in other states or willingness to obtain additional licenses
Work Experience or Related Experience:
• Experience in hospice, palliative care, or oncology settings preferred
• Previous experience working in a call center or remote clinical consultation environment preferred
• Familiarity with pharmacy benefit management (PBM) services preferred
Specialized Knowledge, Skills & Abilities:
• Competency in equianalgesic dosing, generic and therapeutic substitution, formulary management, and prior authorization workflows.
• Strong understanding, competency and adherence to all state and federal pharmacy practice rules and regulations
• Excellent clinical assessment and decision-making skills
• Strong written and verbal communication skills
• Proficiency with typing, computer use, and electronic health record (EHR) systems
• Excellent listening skills and ability to maintain professionalism and empathy
• Strong organizational skills and attention to detail
• Proven ability to manage multiple priorities and projects simultaneously in a fast-paced environment
• Ability to maintain confidentiality, and to exercise discretion and good judgment

Equipment:

Work Environment, Conditions and Demands
• Work is performed in a remote or hybrid office environment.
• Remote work Internet Policy Requirements: To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self-provided internet service of Home or Hybrid Home/Office employees must meet the following criteria:
• At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps is recommended; wireless, wired cable or DSL connection is suggested
• Satellite, cellular and microwave connection can be used only if approved by leadership
• Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
