    Position Summary

    Responsible for the coordination of clinical programs and services for kidney transplant donors and recipients as part of Weill Cornell Medicine’s Transplantation Program.

    Job Responsibilities

    + Coordinates the multidisciplinary screening and evaluation of patients seen by the Weill Cornell Medicine transplantation program at Weill Cornell and Brooklyn Methodist Hospital.

    + The multidisciplinary team includes the transplant nephrologists, transplant surgeons, social work, nutrition, pharmacy, and finance. Coordinators facilitate other medical or surgical specialty visits as needed.

    + Ensures all necessary lab tests, imaging and procedures are performed on potential transplant candidates and that results are updated periodically as required (including bloodwork sent to Rogosin for tissue typing).

    + Participates in making recommendations regarding placing as patient on the transplant waiting list. Educates patients on the benefits of living donation (when applicable).

    + Communicates with all active transplant candidates regarding waiting list status on an ongoing basis. Ensures all medical clearances are updated regularly.

    + On call responsibilities include facilitating organ offers from UNOS. Coordinators also answer patient calls after hours, on holidays and weekends per the on call schedule.

    + Follows patients during their hospitalization and coordinates with transplant team to arrange consulting services as needed. Coordinates outpatient follow up visits with the outpatient transplant team.

    + Educates patients and family regarding all transplant medications, lifestyle changes, as well as outpatient follow-up protocols.

    + Assesses continued recovery of patient through compliance with outpatient visits, scheduling of blood tests, regular review of labs, adjusts immunosuppressive medication as needed during regular lab review.

    + Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations.

    Education

    + Master’s of Science Degree in Nursing

    + Certification in specialty area

    Experience

    Approximately 2 or more years of clinical experience as an NP in a physician practice or hospital setting, preferably within a related specialty.

     

    New graduates with related RN experience may be considered.

     

    Transplant medical knowledge preferred but not required.

    Knowledge, Skills and Abilities

    + Solid foundation of clinical skills (history and physical taking, awareness of medications and potential drug interactions, etc.).

    + Desire to learn and take progressive patient care responsibility.

    + Demonstrated critical thinking and analytical skills.

    + Excellent communication skills (both verbal and written).

    + A team-oriented individual that works collaboratively to achieve team goals.

    Licenses and Certifications

    + NYS Registered Nurse Practitioner (NP) license and current NP registration

    + NYS Registered Professional Nurse license and current RN registration

    + Applicable National Board Certification (such as ANCC, PNCB, etc)

    + Current BLS (must be issued by American Heart Association)

    + Infection Control Certificate

    + Current Narcotics DEA registration certificate
