Job Responsibilities:
As an LPN, you’ll play a vital role in delivering high-quality, person-centered care both primarily in our clinic. Your responsibilities will include:
+ Preparing participants for clinic visits
+ Drawing labs, administering vaccines, and collecting specimens
+ Providing treatments such as wound care and medication setup
+ Supporting participants in the community with proactive health management
+ Assisting with oversight of home health aides and unlicensed staff
You’ll be part of a **collaborative care team** that works together to delay long-term placement and empower seniors to thrive in their own environments.
Required:
+ Current NYS LPN license
+ 1+ year of LPN experience
+ Valid BLS certification (or ability to obtain within 90 days via on-site training)
+ Valid driver’s license and reliable transportation
Preferred:
+ Experience in a Medical Office or Acute Care setting
+ Passion for senior care and a respectful, professional demeanor
+ Strong time management, organizational, and communication skills
Pay Range:$24.00 - $30.10
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
