    Position Summary

    Pine Acres Rehabilitation & Care Center is seeking a Full-Time Physical Therapist (PT) to join our dedicated in-house rehabilitation team. The Physical Therapist is responsible for evaluating, planning, and delivering individualized therapy services to residents recovering from illness, injury, or surgery. This role plays a vital part in helping residents restore mobility, improve strength and balance, and maximize independence in daily living.

    Key Responsibilities

    + Conduct comprehensive physical therapy evaluations to determine patients’ mobility and functional needs.

    + Develop and implement individualized treatment plans that promote strength, endurance, balance, and overall mobility.

    + Provide direct therapy interventions in accordance with physician orders and facility protocols.

    + Educate patients, families, and caregivers on exercise programs, safety, and adaptive techniques to support continued progress.

    + Collaborate closely with interdisciplinary team members—including OT, SLP, nursing, and social services—to ensure coordinated, high-quality care.

    + Maintain accurate and timely documentation in the electronic medical record system.

    + Participate in care plan meetings, clinical discussions, and ongoing professional development.

    + Adhere to all applicable state, federal, and facility policies, procedures, and professional standards.

    + Qualifications

    + Required: Doctorate or Master’s degree in Physical Therapy from an accredited program.

    + Required: Current Iowa Physical Therapist license (or eligibility).

    + Preferred: Experience in subacute, skilled nursing, or long-term care rehabilitation settings.

    + Excellent communication, clinical reasoning, and teamwork skills.
