Position Summary:

We are seeking a licensed Physical Therapist (PT) to join our in-house rehabilitation team. This role will focus on providing high-quality physical therapy services to residents in our subacute rehabilitation unit, helping them achieve their highest level of function and independence. This position offers flexible scheduling with full-time and per diem opportunities available.

Key Responsibilities:

+ Evaluate, plan, and implement individualized physical therapy programs for residents.

+ Provide therapeutic interventions, including mobility training, balance exercises, strengthening, and gait training.

+ Monitor and document resident progress, updating care plans as needed.

+ Collaborate with physicians, nursing staff, occupational therapy, and speech therapy to ensure coordinated care.

+ Educate residents and families on exercises, adaptive techniques, and safe mobility.

+ Participate in team meetings and quality improvement initiatives.

+ Maintain compliance with federal, state, and facility regulations.

Qualifications:

+ Licensed Physical Therapist in the state of Iowa.

+ Minimum of 1 year of experience in a skilled nursing or subacute rehabilitation setting preferred.

+ Strong clinical reasoning, communication, and documentation skills.

+ Ability to work independently and as part of a multidisciplinary team.

+ CPR certification required or willingness to obtain.

Work Schedule:

+ Full-Time: Monday–Friday, standard business hours (flexibility may be required).

+ Per Diem: Flexible scheduling based on facility needs.
