    General Description

    The Employer Resource Connection (ERC) Consultant supports NWSDS’s mission by helpingconsumers of in-home services become effective employers of their Homecare Workers(HCWs). Through the ERC Program, administered in partnership with the Oregon Home CareCommission (OHCC), the Consultant provides education, coaching, and resources thatpromote safe, professional, and person-centered employment relationships betweenconsumers and their HCWs.

     

    Essential Functions:1. Promotes the ERC and ADRC programs2. Evaluates consumer needs and develops service plans3. Supports participants’ success with program and in-home services4. Protects consumers and reduces agency risk5. Demonstrates professionalism, customer service, and agency representation

     

    1. Promotes the ERC and ADRC Programs? Understands and communicates the objectives, values, and benefits of the ERC andADRC programs.? Conducts outreach and education for potential participants, community partners,and referral sources. Delivery includes public speaking to audiences of all sizes.? Promotes the program’s availability and benefits through presentations, meetings,and resource sharing.? Builds collaborative relationships with Case Managers, community agencies, andpartners to support referrals and participation.? Maintains current knowledge of ADRC and OHCC resources, policies, and regionalservice networks.

    2. Evaluates Consumer Needs and Develops Service Plans? Reviews and processes referrals from Case Managers and self-referrals in a timelyand professional manner.? Conducts Planning Interviews (PI) or Consumer Checklists to determine readinessand ability to manage a Home Care Worker.? Collaborates with Case Managers, family members, and other representatives toidentify barriers to effective consumer-employer management.? Develops individualized Plans for ERC Services (PSS) that respect consumer choicewhile addressing identified needs.? Provides direct coaching and technical support to help consumers recruit, hire,train, and supervise Homecare Workers.? Assists participants with creating Carina registry profiles, preparing interviewquestions, and conducting reference checks.? Monitors consumer progress and provides follow-up support to ensure sustainableservice success.

    3. Supports Participants’ Success with Program and In-Home Services? Provides one-on-one and group workshops to strengthen consumer-employers’skills in managing Homecare Workers.? Helps participants establish expectations, communicate effectively, and maintainprofessional boundaries.? Offers coaching to resolve performance issues or interpersonal conflicts betweenconsumers and workers.? Supports adherence to the consumer’s service plan to ensure person-centered,equitable, and safe service delivery.? Encourages consumer independence and confidence through education and accessto ongoing resources.

    4. Protects Consumers and Reduces Agency Risk? Complies with program rules, agency policies, and state and federal documentationrequirements.? Accurately records all service activities, plans, and outcomes in Oregon ACCESS,Carina, and other required systems.? Submits reports, statistics, and invoices according to agency and OHCC contractstandards.? Maintains confidentiality and data security in accordance with HIPAA and agencypolicy.? Ensures consumers understand their obligation to provide a safe, respectful, andharassment-free workplace.

    5. Demonstrates Professionalism, Customer Service, and Agency Representation? Builds confidence in the ERC program and agency through consistent follow-throughand clear communication.? Demonstrates empathy, respect, and cultural awareness in all interactions.? Works collaboratively across teams and functions to support program success andservice quality.? Uses technology effectively, organizes work efficiently, and meets deadlinesconsistently.? Maintains a positive and solutions-oriented attitude that reinforces the agency’smission and service philosophy.

     

    Required Knowledge, Skills, and Abilities? Knowledge of Oregon in-home service rules, ADRC principles, and Home CareCommission programs.? Ability to present to small and large groups, tailor content to diverse audiences,explain complex information in plain language, and manage Q&A effectively.? Strong understanding of person-centered and trauma-informed service approaches.? Ability to assess needs, develop service plans, and facilitate agreements amongmultiple stakeholders.? Excellent communication, coaching, and conflict-resolution skills.? Ability to document thoroughly and maintain accurate, confidential records.? Proficiency in systems such as Oregon ACCESS, CAPS, Carina, Microsoft Office, andvirtual platforms.? Capacity to work independently while maintaining accountability and professionalism.?     Required Experience:

    Minimum QualificationsA qualified applicant will have a minimum of four (4) years of equivalent combination ofeducation and/or experience which demonstrates the knowledge, skills and abilities requiredto perform the essential functions of this position. Minimum qualifications may be metthrough any of the following:? Bachelor’s degree in social sciences, human services, or a closely related field; or? Four (4) years of relevant experience providing direct service to consumers inorganizational settings (e.g., schools, public agencies, healthcare, or communityorganizations); or? Any combination of related education and work experience equally at least 4 years.

     

    Special Requirements? Background Check. All positions must pass a basic criminal history check. Conviction ofa crime may not necessarily prohibit employment. Some may require additionalclearance, including but not limited to national or state fingerprinting.? Driving for Agency Business. Applicants must possess a current driver's license in theirstate of residence and acceptable driving history. May require proof of personalautomobile liability insurance [ORS Chapter 806] and use of a personal vehicle forbusiness use.(For Bilingual positions only)• Successful completion of a language proficiency test.

     

    Work Environment and Physical RequirementsEmployer Resource Connection Consultants work both in an office environment and in thecommunity.? Work is performed in an office setting, consumer homes, health care facilities, andother community locations.? Regularly required to sit, stand, walk, reach, and use hands to handle materials oroperate equipment.? Must be able to lift up to 20 pounds.? May encounter individuals with contagious conditions, challenging behaviors, ordomestic animals.? Frequent use of computers, telephones, and other standard office equipment.This job will typically be conducted during regular business hours, but may require additionalor irregular hours on occasion.The physical demands are representative of those that must be met by an employee tosuccessfully perform the essential functions of this job.
