    Edward M. Kennedy Community Health Center is one of the largest community health centers in Massachusetts serving Worcester, Framingham, Milford, and the surrounding communities.  We are a thriving and growing organization, and our team is expanding across sites to support this growth.

     

    We are currently hiring a Community Health Worker – Behavioral Health based in Worcester. As part of the care management team, the Community Health Worker (CHW) works as a part of the health care team to provide comprehensive, coordinated CHW interventions in accordance with the Massachusetts Board of Certification of Community Health Workers core competencies as part of a patient centered health care home for Health Center patients.

    Essential Functions:

    + Provides health system education and advocacy to help patients navigate care and access needed resources.

    + Supports patients facing mental health, emotional, or substance-use challenges with culturally appropriate connections to services.

    + Conducts SDOH assessments and creates intervention plans based on identified needs.

    + Assists patients with housing, transportation, education, employment, child care, food security, and utilities.

    + Follows up with referred patients to address social barriers affecting engagement in care and treatment adherence.

    + Participates in population-based projects and meets grant-related requirements.

    + Performs other duties as assigned.

    Required Qualifications:

    + Minimum 1 year of work or volunteer experience in human services or healthcare with demonstrated knowledge of or ability to work with the targeted community.

    + Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.

    + Excellent written and oral communication skills.

    + Successful completion of the CHW core competency training within 2 years of employment.
