    We are currently seeking a*Violence Intervention Case Manager*to join the Next Step program. This full-time role will primarily work on-site/in-person. This is a grant funded role. A valid driver's license and access to an insured, reliable vehicle are required.

     

    Next Step is a hospital-based violence intervention program partnership between the City of Minneapolis and Hennepin Healthcare System (HHS). The program serves survivors of violent injury (e.g. gunshot, stabbing, serious assault) who have been treated at HHS (HCMC), North Memorial, Children's Minneapolis, or Abbott-Northwestern. The staff provide inpatient services and resources to help the participant address his/her/their traumatic injury and recovery and provide services to help the participant develop a Safety Care Plan.

     

    *_SUMMARY_*

     

    Under general supervision, provides service to patients affected by violent crimes. Provides information, referrals and resources for clients. Assists in the care of complex patients to facilitate the delivery of cost effective healthcare and in the identification of appropriate utilization of resources. Coaches caregivers in developmentally appropriate positive style to provide care. Refers education and outreach program activities, including implementation of appropriate policies and procedures and evaluation of program and services. Works collaboratively with interdisciplinary staff internal and external to the organization to promote health and healing.

    *_RESPONSIBILITIES_*

    * Explain policies about confidential services to family members during the first visit

    o Round on patients while they are inpatient

    * Provide education to caregivers and survivors about conditions that may impact trauma

    * Provide coaching to caregivers and clients for their identified supports on health issues and client identified goals

    o Refer clients to appropriate health resources and social services and support clients in accessing resources, particularly in regards to parents/caregivers

    * Provide positive developmentally appropriate coaching in individual and group settings under the supervision of management

    o Work with survivor to develop safety plan

    o Facilitate meetings to identify appropriate visitors and or stakeholders for patients we serve

    o Provide support on safety planning, referring emergency assistance programs and or emergency shelter or housing

    o Develop goal plan with assistance from client

    * Provide information and protect victim rights and support client with outside resources and agencies

    * Actively refer services and follow-up with patients of high risk; work collaboratively with client and family who are involved in the care of the client

    o Order transportation for patients to be able to utilize supports/resources

    o Assist with relocation of clients

    * Perform face-to-face and phone contact with a specified number of patients on a scheduled basis and as needed

    * Develop and establish relationships with other community and social support providers to aid in effective referrals and timely access to services

    * Collect and track data for entry into the database

    * Participate in scholarly activities, presentations, train-the-trainer opportunities

    * Assist clients with financial stressors under supervision of management

    * Other duties as assigned, but only when trained and qualified to do so

    *_QUALIFICATIONS_*

    /*Minimum Qualifications:*/

    * Post high school education

    * Two (2) years experience in program development, community health and/or grant development

    * Experience working with youth/young adults and families

    * Valid MN Driver's license

    -OR-

    * An approved equivalent combination of education and experience

    * Skill in youth/family engagement and referral

    * Basic computer skills

    * Knowledge of diversity issues; grief and loss; social, economic, legal and medical systems; safety

    * Ability to confront discrimination, prejudice, and fear

    * Ability to work within medical hierarchy

    * Ability to keep accurate and timely records

    * Ability to assist in complex problem-solving with clients

    * Ability to work autonomously, network with related agencies, build coalitions, and establish boundaries with clients, staff and agencies

    * Ability to be flexible and adapt to changing priorities

    * Must demonstrate effective written and oral communication

    * Ability to deal with death and difficult issues sensitively

    * Ability to verbally de-escalate hostile situations

    * Ability to manage personal differences for the strength of the team
