Patient Navigator

Schedule: Monday–Friday, 8:00 AM – 4:30 PM

Pay: $29.00/hr

Duration: Approximately 6 months

About the role

+ Support patients throughout their care journey.

+ Requires strong communication skills, adaptability, and ability to manage multiple priorities in a fast-paced environment.

Minimum Qualifications

+ High School diploma or GED required; Associate’s degree preferred.

+ At least 2 years of experience as:

+ Medical Office Referral Coordinator

+ Medical Assistant

+ Health Plan role

+ Managed Care Organization Utilization Technician experience preferred.

+ Basic understanding of:

+ ALOS

+ Readmission rates

+ Gaps in care

+ Proficiency in Microsoft Office, typing, and data entry.

+ Ability to communicate clearly and professionally with individuals at all levels—even in stressful situations.

+ Flexible and adaptable to changing patient and organizational priorities.

+ Skilled in managing conflict, stress, and multiple demands effectively.

+ Demonstrates respect, concern, and empathy for patients’ spiritual and emotional needs.

The Navigator is a key member of the Care Coordination team, working to improve care quality and outcomes for patients with complex needs. This role coordinates care across the healthcare system through collaboration with multidisciplinary teams, helping reduce fragmentation, improve compliance, and remove treatment barriers. The Navigator assists patients in navigating their care to reduce hospitalizations, readmissions, and ER visits. Responsibilities include telephonic management and direct patient contact through clinic follow-ups, community visits, and home visits as needed. Travel may be required, with telecommuting options available.
