    What We Offer

     

    Clinical Service Navigator (Registered Nurse)

     

    + Schedule: 40 hours per week, Monday through Friday 8-5pm

    + Location: Forsyth Medical Center

    + Patient Population: COPD and Heart Failure Patients

     

    Serves as primary liaison between the patient and his/her medical team as well as patient advocate and support. Provide referral sources and support services to patients to work towards lowering patients' readmissions.

    What We're Looking For

     

    + Education: 4 Year / Bachelors Degree, preferred.

    + Experience: Three to five years of nursing experience, required. Two years of experience in care of targeted patient population; experience in a broad range of clinical situations or settings, preferred.

    + Licensure/Certification : RN, required.

    + Additional Skills/Requirements (required): Ability to successfully complete generic and department-specific skills validation and competency testing.
