    Program: St. Nicholas House

     

    Salary: $4 3 ,000 - $4 7 , 5 00 per year

    Program Description

    St. Nicholas House is a supportive SRO providing permanent housing to 94 single adults, including 40 men and women eligible under the NY/NY II Agreement, and 21 men and women recovering from substance abuse referred directly from the NYC Department of Homeless Services shelter system. The building also offers affordable studio units to 3 3 low-income community members.

    Overall Responsibilities

    Under the general direction of the Clinical Director, the Case Manager is responsible for providing assessments, case management, and short-term counseling to clients, enabling them to maintain housing, live productively in the community, manage mental illness, sustain substance abuse recovery, maintain stable physical health, and independently manage finances and entitlements.

     

    Essential Duties and Responsibilities

    The essential duties of the Case Manager include, but are not limited to:

    + Conduct pre-intake interviews to determine appropriateness for housing at St. Nicholas House.

    + Complete intakes, assessments, and psychosocial evaluations in accordance with agency standards.

    + Efficiently utilize the AWARDS digital case management system to input and update all client information and documentation.

    + Provide case management services, supporting the creation and implementation of individualized Service Plans with both short- and long-term goals.

    + Participate in clinical team meetings to discuss high-risk clients and update tracking spreadsheets.

    + Provide short-term supportive counseling and advocacy on behalf of tenants.

    + Organize and facilitate support groups, tenant activities, and events both on- and off-site.

    + Maintain communication with internal and external service providers on behalf of clients.

    + Identify service needs and make referrals for medical, mental health, psychiatric, substance use, and other supportive services.

    + Escort clients to essential appointments and meetings when necessary to ensure proper follow-through.

    + Perform other duties as assigned by the Clinical Director or Program Director.

    Qualifications

    + Bachelor’s degree in social work, psychology, or another human services field from an accredited college or university.

    + Minimum of 1–2 years of experience working with homeless individuals, individuals with mental illness, or other special needs populations preferred.

    + Strong teamwork, organizational, interpersonal, communication, and computer skills; bilingual abilities are a plus.

    + Ability to work cooperatively with peers and staff to effectively and efficiently meet client needs .

    + Ability to work independently and adjust one’s schedule to accommodate client needs.

    + Excellent oral, written, and listening communication skills.

    + Ability to effectively interface with clients and staff at all levels.

    + Proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.
